摘要
目的探讨前路复位内固定治疗下颈椎脱位并关节突交锁。方法对26例下颈椎骨折脱位合并关节突交锁患者(单侧小关节交锁10例,双侧小关交锁16例)行前路复位植骨融合加钢板内固定术。结果术中无大血管、气管、食管、脊髓损伤。26例均获随访,时间5～6个月。患者复位良好,颈椎椎间高度和生理曲度维持良好,脊髓损伤患者术后神经功能均有1～2级恢复。结论前路复位内固定治疗下颈椎滑脱并关节突交锁疗效确切,术中只要掌握好复位技巧,能解决脱位和交锁的问题。
Objective To evaluate the effect of the anterior reduction and internal fixation of fresh dislocation and facet locking of lower cervical spine. Methods 26 patients＇ clinical data with lower cervical facet fracture and dislo- cation locking were retrospectively analyzed. There were unilateral facet interlocking in 10 cases, bilateral locking in 16 cases. All patients underwent anterior decompression and fusion and plate fixation. Results There was no intrao- perative large blood vessels, trachea, esophagus, spinal cord injury. Mean follow-up were 5 to 6 months. All patients got good reduction, and restoration of intervertebral height and cervical lordosis to maintain good neurological function of spinal cord injury with 1 ~ 2 grade recovery. Conclusions Anterior reduction and internal fixation of fresh slip- page and lower cervical facet lock is effective, as long as the master reset intraoperative techniques, which can solve the problem of dislocation and locking.
