超声引导下定位导丝辅助在临床触诊阴性的乳腺病灶切除的应用及意义 被引量:12

摘要 目的探讨超声引导下定位导丝辅助在临床触诊阴性的乳腺病灶切除的应用价值。方法对2009年9月~2011年10月35例47个触诊阴性的乳腺病灶在超声引导下穿刺病灶并用金属导丝标记定位,切除病灶,明确病理诊断。结果定位全部成功,定位操作时间3~7 min,平均4 min,且无并发症出现。35例47个病灶经超声引导下导丝定位后全部准确切除。术中冰冻病理诊断:浸润性导管癌1例,随即在全身麻醉下行根治手术;乳腺导管内癌1例,随即行单纯乳腺切除术;乳腺纤维腺瘤15例;乳腺病伴纤维腺瘤形成7例;乳腺病增生结节5例;乳腺囊肿4例;乳管内乳头状瘤2例。冰冻切片病理检查与术后石蜡病检结果一致。结论超声引导下定位导丝辅助临床触诊阴性的乳腺病灶切除定位较准确,对良性病变可缩小手术切除范围,对恶性病变可早发现,是一种简单、准确、安全及实用的方法。 Objective To explore the efficacy of ultrasound-guided wire localization in the resection of nonpalpable breast lesions. Methods A retrospective study on 35 patients with non-palpable breast lesions (47 lesions) who received ultrasound- guided wire localization for resection in our hospital from 2009 September to 2011 October. Results The localization was successfully completed in all the patients within a mean of d min (ranged from 3 to 7 ). No complications occurred during the procedure. All the 47 lesions were removed accurately after the localization. Intraoperative fast pathological examination showed infiltrative ductal carcinoma in 1 case (radical operation was carried out immediately under general anesthesia), breast ductal carcinoma in 1 case (simple mastectomy was made), breast fibroadenoma in 15 cases, breast disease complicated with fibroadenoma formation in 7, hyperplastic nodules in 5, breast cyst in 4, and intraductal papillary epithelioma in 2. Postoperative pathological examination was identical to the fast pathological results. Conclusion Ultrasound-guided wire localization provides accurate position of nonpalpable breast lesion, and thus can reduce surgical extension for benign tumors, and identify malignancies in an early stage.
