摘要
目的讨论经闭孔经阴道无张力尿道中段悬吊术(TVT-O)治疗女性压力性尿失禁的疗效。方法选择我院收治的38例女性压力性尿失禁患者,通过回顾TVT-O治疗过程,使用Grouts-Blaivas评分法进行疗效评价,了解TVT-O治疗女性压力性尿失禁的效果。结果 38例患者均成功完成手术,手术成功率为100%。手术时间20～45min,平均手术时间32.6min。术中出血量15～110mL,平均出血量57.3mL。患者无血管、膀胱、尿道以及神经损伤。24～72h后拔除尿管,患者压力性尿失禁症状消失,未出现排尿不适情况。经过检查,患者无明显的感染、水肿、出血以及排异反应等。患者出院时间从3～10d不等,平均出院时间5.7d。经过3～18月的随访,患者治愈35例,改善良好2例,改善中等1例,无改善差及失败患者。手术有效率达到100%,手术治愈率92.1%。结论 TVT-O作为TVT的改良治疗措施,在女性压力性尿失禁治疗方面具有重要的临床价值,值得临床推广与运用。
Objective To discuss the trans-obturator tension-free vaginal tape （TVT-O） in the treatment of female stress urinary incontinence. Methods 38 cases of female patients with stress urinary incontinence treated with TVT-O in our hospital were selected. The treatment process of TVT-O was analyzed. With the follow-up of 3 to 18 months, the Grouts-Blaivas score assessment was used to understand the effect of TVT-O in the treatment of female stress urinary incontinence. Results All 38 patients successfully completed the operation, operation success rate was 100%. The average operation time was 32.6 rain （20 - 45 rain）. The amount of bleeding in operation was 15 - 110 mL, average bleeding volume of 57.3 mL. Patients had no vascular and nerve injury, bladder, urethra. After the catheter removal 24 - 72 h in patients with stress urinary incontinence, the symptoms disappeared, without any discomfort on urination situation. After examination, the patients showed no infection, edema, hemorrhage and rejection. Patients were discharged ranging from 3 to 10 d, the average discharge time was 5.7 d. After 3 to 18 months of follow-up, 35 cases were cured, 2 cases improved well, one case improved mild, without no improvement and failure patients. The operation results showed the effective rate of 100%, and the cure rate of 92.1%. Conclusions TVT-O as TVT improved treatment measure, has important clinical value in the treatment of female stress urinary inconti nence, which is worthy of clinical promotion and application.
