摘要
目的探讨在基层医院应用传统器械经脐单孔腹腔镜胆囊切除术的适应证、技术难点及可行性。方法回顾性分析2011年7月至2012年7月21例胆囊良性疾病患者使用传统器械行经脐单孔腹腔镜胆囊切除术的临床资料,包括手术适应证、手术方式、手术时间、术中出血、术中技巧、术后并发症及门诊随访等。结果 21例患者在传统器械下经脐单孔腹腔镜胆囊切除术均顺利完成手术。平均手术时间55.6 min,术中平均出血15 mL,术后平均住院时间3 d,均无胆漏、出血、脐疝等并发症;术后随访脐部切口瘢痕不明显,美观,效果满意。结论传统器械下经脐单孔腹腔镜胆囊切除术安全、可行,近期疗效满意;在掌握适应证的前提下,可在基层医院进一步开展。
Objective To evaluate the indications, technical difficulties and feasibility of transumbilical single port laparoscopic cholecystectomy by traditional instruments. Methods The clinical data of 21 patients with transumbilical single port laparoscopic cholecystectomy by traditional instruments from July 2011 to July 2012 were retrospectively analayzed on the operative indications,operation patterns,operative time,intraoperative blood loss,intraoperative techniques,postoperative complications and clinic follow up. Results Transumbilical single port laparoscopic cholecystectomy by traditional instruments was com- pleted successfully in 21 cases, no case died of the operation. The average operative time was 55.6 min,the mean blood loss was 15 mL and the mean postoperative hospitalzation time was 3 d. No complications such as biliary leakage ,hemorrhage and umbili- cal hernia occurred. Abdominal incision scar was inapparent with good appearance and satisfactory effects in postoperative follow up. Conclusion Transumbilical single port laparoscopic cholecystectomy by traditional instruments is safe and feasible with sat- isfactory short-term effects ,which may be further carried out in primary hospitals under the premise of grasping the indications.
