地左辛用于腹腔镜胆囊切除术中的临床观察

摘要 目的观察地左辛对腹腔镜胆囊切除术(LC)患者的影响。方法 100例择期LC患者按随机数字法分为地左辛组(D组)和生理盐水对照组(C组),每组50例。手术开始时,地左辛组静脉注射地左辛5 mg,对照组静脉注射生理盐水1 mL。观察两组患者手术时间、睁眼时间、拔管时间和麻醉前、气腹后10 min、拔管时、拔管后10 min、拔管后20 min的SBP、DBP、HR、SpO2以及术后躁动情况,对两组患者拔管后即刻、5 min、10 min、20 min、60 min进行VAS评分。结果 D组患者气腹后10 min、拔管时、拔管后10 min的SBP、DBP值均明显低于C组(P<0.05),D组术后躁动发生率明显低于C组(P<0.05),拔管后5 min、10 min、20 min、60 min D组患者VAS评分明显低于C组(P<0.05)。结论 LC术中使用5 mg地左辛,血流动力学更稳定,能有效缓解术后疼痛,术后躁动发生率显著降低。 Objective To observe the effect of dezocine to LC patients. Methods One hundred cases of selective LC patients were randomly divided into dezocine group (D group) and physiological saline group (C group), 50 cases in each group. When the operation began, dezocine group was intravenous injected with 5mg dezocine, C group was injected with 1 mL physiological saline. Two groups were observed in patients of SBP, DBP, HR, SpO2 and postoperative restlessness in operation time, sober time, extubation time and before anesthesia, 10 minutes after pneumoperitoneum, extubation, 10 minutes after extubation , 20 minutes after extubation; VAS score in patients of two groups immediately after extubation, 5 minutes, 10 min, 20 rain and 60 min after extubation were made. Results SBP, DBP values of D group were significantly lower than that in C group (P 〈 0.05) in extubation time, 10 minutes after pneumoperitoneum, 10 rain after extubation. In D group, postoperative restlessness rate was obviously lower than group C (P 〈 0.05), VAS score in patients of D group were significantly lower than that of group C (P 〈 0.05) in 5 minutes, 10 minutes, 20 minutes, 60 minutes after extubation. Conclusion LC intraoperative use of 5 mg dezocine, hemodynamic stability strengthen, can effectively alleviate postoperative pain, postoperative restlessness rate was significantly lower.
