摘要
目的:探讨重症胸、腹部损伤的早期诊治方法。方法:收集2006年2月-2010年9月收治63例重症胸、腹部损伤患者,皆合并不同程度的休克,其中27例经胸或腹腔穿刺抽出较多不凝固血,30例经螺旋CT检查确诊,6例经B超检查确诊,紧急手术治疗60例,3例12h内明确诊断后手术治疗。其中胸部损伤15例、腹部损伤31例、胸腹联合伤17例。结果:58例救治成功,救治成功率92%。并发症10例,其中脓毒症1例,多器官功能障碍综合征(MODS)3例,DIC2例,肺部感染2例,切口感染2例;5例死亡,其中4例于伤后12h内死亡,主要原因为多脏器严重损伤及失血性休克。结论:对重症胸、腹部损伤患者采用创伤急救体系信息化模式化管理,在最短时间内进行医学干预、快速评估、准确诊断,对损伤器官针对性的处理;提高生存率、降低致残率。
Objective：To explore early methods of diagnosis and treatments for severe thoracic and abdominal injuries.Method：Through collecting 63 severe thoracic and abdominal injured patients,each with different degree of shock and were diagnosed and treated from Feb.2006 to Sep.2010,of which 27 cases were diagnosed because more no solidify blood was drawn out through thoracocentesis or peritoneocentesis,30 cases diagnosed by spiral CT,6 cases diagnosed by B-ultrasound,60 cases of emergency surgical operation,3 cases surgical operated within 12 hours after a definite diagnosis.And in these cases,15 cases were thoracic injuries,31 cases were abdominal injuries,17 cases were thoracoabdominal injuries.Result：58 cases were successfully cured,with a 92% cure rate;10 cases developed complications,including 1 Sepsis,3 MODS,2 DIC,2 Pulmonary infection,2 incision infection;5 cases died,of which 4 cases died within 12 hours,because of severe multiple organs damage and hemorrhagic shock.Conclusion：The analysis of the clinical cases show that in the shortest possible time implementing Trauma-emergency-system-management informatively and modeling to severe thoracic and abdominal injured patients,conducting medical intervention,rapid assessment,accurate diagnosis,targeted treatment to injured organ can obviously increase the survival rate and reduce the rate of disability.
