骨形态发生蛋白和转化生长因子-β及碱性成纤维细胞生长因子在骨折修复中的表达 被引量:22

Experimental study of expression of BMP, TGF-β and bFGF in fracture healing

摘要 目的 观察骨形态发生蛋白 (BMP)、转化生长因子 - β(TGF - β)、碱性成纤维细胞生长因子 (bFGF)在骨折修复中的表达及分布情况 ,进而探讨其作用机制。方法 以成年雄性新西兰兔为研究对象 ,制作桡骨骨折愈合模型。伤后不同时期处死取材 ,分别进行组织学和BMP、TGF - β和bFGF免疫组化染色观察。结果  (1)伤后 3d开始形成原始骨痂。 1周时肉芽组织中的间质细胞开始分化为软骨细胞 ,软骨形成后再进行软骨内化骨。 4周时形成连接骨折端的桥接骨痂。 (2 )伤后不同时期 ,血肿中炎性细胞表达bFGF。骨膜增殖细胞、骨端骨细胞以及原始骨痂成骨细胞表达TGF - β。肉芽组织中间质细胞、单核巨噬细胞和多核巨细胞表达TGF - β、bFGF。原始骨痂成骨细胞、骨端骨细胞和软骨细胞表达BMP。表达后BMP、TGF - β较均匀分布于软骨基质以及肉芽组织间质中 ,bFGF主要分布于肉芽组织中非活跃的间质细胞周围。结论 BMP、TGF - β和bFGF有着各自的表达和分布特点 ,并共同调解骨原细胞的增殖和成骨细胞、软骨细胞的分化 ,最终完成骨折修复。 Objective To observe the expression and distribution of bone morphogenic protein (BMP), transforming growth factor-β (TGF-β) and basic fibroblast growth factor (bFGF) in fracture healing so as to find out their action mechanism. Methods Adult male New Zealand rabbits were used to make fracture healing model in the experiment. After operation, rats were killed in different time and specimens were possessed for histological observation and immunohistochemical observation on staining of BMP, TGF-β and bFGF. Results (1) Primary callus came into being 3 days after operation. Mesenchymal cells from peripheral tissues around the primary callus were differentiated into chondrocytes 1 week later. Endochondral ossification was going on then. Bridging callus was formed in the fracture sites 4 weeks after operation. (2)In different time after operation, inflammatory cells in hematoma expressed bFGF. Expression of TGF-β was from proliferative cells in periosteum, bone cells in fracture ends and osteoblasts in the primary callus.Expression of TGF-β and bFGF was from mesenchymal cells, macrophages and multinuclear giant cells in the granulation tissue. Osteoblasts in the primary callus, bone cells in fracture ends and chondrocytes expressed BMP. After the expression completed, BMP and TGF-β were distributed in the chondral matrix and granulation tissues. The bFGF was mainly distributed around non-active mesenchymal cells in the granulation tissues. Conclusions BMP, TGF-β and bFGF possessed their own different characteristics of expression and distribution and cooperatively modulated proliferation of osteogenetic cells and differentiation of chondrocytes and osteoblasts for fracture healing.
