摘要
目的 探讨自身免疫性耳聋的机理及有效的诊断方法。方法 取胎龄为 4~ 7月的膜迷路制备人类胚胎耳蜗抗原 (HumanembryocochlearAg ,HEC -Ag)。取HEC -Ag液为兔子做皮内注射 ,留取血清作阳性血清对照。健康兔血清作阴性血清对照。收集 76例患者的血清为耳聋组 ,34例正常健康人血清为对照组。对血清进行HEC -Ab的检测。结合临床疗效分析。结果 耳聋组光密度值为 0 .16 4± 0 .0 58,对照组光密度值为 0 .0 6 3± 0 .0 31(P<0 .0 1)。在耳聋组中 13例阳性 (17.1% ) ,2例强阳性 (2 .6 % ) ,阳性率为 19.7%。在对照组中 2例阳性 ,阳性率为 5.9% (P <0 .0 1)。结论 提示HEC -Ab的检测可作为自身免疫性耳聋诊断的初步筛选方法。
Objective To study the mechanism and effective diagnosis methods of autoimmune sensorineural hearing loss. Methods The membranous labyrinth of human embryo (4~7 months) was taken out. Human Embryo Cochlear Antigen (HEC-Ag) was prepared. The rabbit was injected with HEC-Ag. A month later, its blood was taken as positive control serum. The blood of a healthy rabbit was taken as negative control serum. Seventy-six patients formed the sensorineural hearing loss group and 34 healthy people were in the control group. The HEC-Ab in the serum was tested. The mechanism and effective diagnosis methods of autoimmune sensorineural hearing loss were discussed in combination with the clinical results. Results The absorbency (A) of positive control serum in the rabbit was 0.103±0.034 and the A of negative control serum in the rabbit was 0.077 ± 0.012 . The A in the experimental group was 0.164 ± 0.058 and in the control group it was 0.063 ± 0.031 (P< 0.01 ). Thirteen cases were positive in the experimental group ( 17.1% ) and 2 were strongly positive ( 2.6% ). The positive rate was 1 9.7% . Two cases were positive in the control group ( 5.9% ) (P< 0.01 ). Conclusion The HEC-Ab test is a filtration method in the diagnosis of autoimmune sesorineural hearing loss.
