摘要
目的探讨胰腺常见囊性病变的CT表现特征。方法结合临床病史及病理类型,回顾性分析2009年9月至2012年3月间共31例胰腺囊性病变患者的CT影像资料,其中包括假性囊肿14例,真性囊肿2例,浆液性囊腺瘤4例,黏液性囊腺瘤4例,黏液性囊腺癌2例,胰腺实性假乳头状瘤3例,胰腺癌2例1例伴明显液化坏死,均经手术病理证实。结果胰腺假性囊肿12例发生于胰周,2例发生于胰腺实质内,呈圆形或椭圆形囊状低密度影;真性囊肿2例均发生于胰腺实质内,呈多囊状改变;囊腺瘤8例均位于胰腺体尾部,浆液性呈类圆形囊状低密度影,黏液性囊较大,可见条状分隔及斑点状钙化影;黏液性囊腺癌2例位于胰体部,可见壁结节;实性假乳头状瘤2例位于胰腺体部,1例位于胰尾,均以囊性成分为主,边缘可见条片状软组织密度影及斑点状钙化影;胰腺癌2例,1例位于胰头部,坏死囊变区内见索条状软组织成分,1例位于胰颈部。结论胰腺囊性病变的CT表现有很多共性,也有其各自的特征性表现,多层螺旋CT(MSCT)可以显示病变内部的细微结构,对胰腺囊性病变的有较高的诊断及鉴别诊断价值。
Objective To discuss the multi-slice spiral CT features of common pancreatic cystic lesions.Methods Combined with clinical history and pathological types,CT data of 31 patients with pancreatic cystic lesions from hospital between September 2009 and March 2012 were analyzed retrospectively,which included 14 cases of pseudocyst,2 cysts,4 serous cyst adenoma,4 mucinous cyst adenoma,2 mucinous cystadenocarcinoma,3 solid pseudopapillary tumor of pancreas（SPT）,and 2 pancreatic cancer.All theses cases were surgically and pathologically confirmed.Results Of 14 cases of pseudocyst,12 were found in the peri-pancreas,2 in the parenchyma of the pancreas,appearing as hypodense shadow of circular or elliptical cystic mass.2 cysts were found in the parenchyma of the pancreas,appearing as polycystic state.8 cystadenoma were located on the pancreatic tail,with serous cystadenoma appearing as hypodense shadow of semicircular cystic mass,with larger mecinous cyst,and with striped partition and punctuate calcified shadow.2 mucinous cystadenocarcinoma were found in the pancreatic mass with mural nodule.2 solid pseudopapillary tumor of pancreas（SPT） were found in the pancreas and 1 in the pancreatic tail,both of which showed characteristics of cyst,striped soft tissue,and punctuate calcified shadow.Of 2 pancreatic cancers,1 was found in the head of pancreas with streak soft tissue in the necrotic cyst,and 1 in the neck of pancreas.Conclusion CT features of different pancreatic cystic lesions have something in common,but still with some respective features.Multi-slice spiral CT can show the subtle features of the pancreatic cystic lesions,and also is important in diagnosing and differentiating pancreatic cystic lesions.
