摘要
目的回顾性分析不同妇科手术方式医院感染率和感染部位分布,比较微创妇科手术与传统手术方式的医院感染情况,为合理选择妇科手术方式提供依据。方法 2006年10月-2011年10月妇科手术26 161例临床资料,选取子宫肌瘤摘除术924例,比较不同手术方式的医院感染情况,计数资料采用χ2检验。结果传统妇科手术方式医院感染率为15.72%,明显高于微创妇科手术方式;手术后医院感染部位中泌尿道感染率为1.08%,感染构成比占44.83%,明显高于其他感染部位(P<0.05),各感染部位在开腹手术中的发生率明显高于微创妇科手术方式(P<0.05);子宫肌瘤患者行开腹手术医院感染率为11.42%,显著高于微创妇科手术方式(P<0.05)。结论微创妇科手术医院感染率明显低于传统妇科手术方式,各感染部位在开腹手术中的发生率明显高于微创妇科手术方式。
OBJECTIVE To respectively analyze the incidence of nosocomial infections and the infection sites due to the different gynecologic surgeries and compare the infection status of the minimally invasive gynecological surgery and the traditional surgery so as to provide basis for the reasonable selection of gynecological surgery.METHODS The clinical data of 26 161 patients undergoing gynecologic surgeries were analyzed from Oct 2006 to Oct 2011,and 924 patients who underwent the hysteromyomectomy were selected as the study objects,the incidence rates of the nosocomial infections were compard between the various surgeries,Chi-square test was conducted for the statistics.RESULTS The nosocomial infection rate of the traditional gynecologic surgery was 15.72%,significantly higher than that of the minimally invasive gynecologic surgery.The urinary tract infection rate was 1.08% after the surgery with the constituent ratio of 44.83%,significantly higher than that of other inflection sites（P0.05）.The incidence of the infections was significantly higher in the laparotomy than in the minimally invasive gynecological surgery（P0.05）;the incidence of nosocomial infections in uterine fibroids patients undergoing laparotomy was 11.42%,significantly higher than that of the minimally invasive gynecological surgery（P0.05）.CONCLUSION The incidence rate of nosocomial infections is significantly lower in the minimally gynecological surgery than in the traditional gynecologic surgery.The incidence of various site infections is significantly higher in patients undergoing laparotomy than those undergoing the minimally invasive gynecological surgery.
