摘要
目的:观察柔肝健脾活血利水法联合西药治疗肝硬化的临床疗效。方法:将97例肝硬化患者随机分为两组,治疗组49例予以中医应用柔肝健脾活血利水法联合西药常规治疗,对照组48例予以西药常规对症处理,观察临床疗效、肝功能及肝纤维化指标的改善情况。结果:总有效率方面,治疗组为95.92%,对照组为75%。两组总有效率比较,差异有非常显著性意义(P<0.01)。治疗组治疗后肝功能、B超检查指标与治疗前比较,差异有非常显著性意义(P<0.01)。治疗组各项肝纤维化指标治疗后与治疗前比较,差异有显著性意义(P<0.05)。治疗组在改善总胆红素(TBil)、血清白蛋白(Alb)、门静脉主干内径(PVID)、血清透明质酸酶(HA)、Ⅳ型胶原(IVC)、层粘连蛋白(LN)方面较对照组有显著差异(P<0.01,P<0.05)。复发率方面,治疗组6和12个月分别为17.14%和42.86%;对照组分别为40.54%和83.78%。两组比较,差异有非常显著性意义(P<0.01)。结论:柔肝健脾活血利水法联合西药治疗肝硬化可以明显改善患者的临床症状、体征,疗效确切,提高了临床疗效,且远期疗效较好,从而提高了患者的生活质量。
Objective：To explore the efficacy of western medicine in combination with principles of emolliating liver, in- vigorating spleen,promoting blood flow and eliminating excess fluid on treating cirrhosis. Methods：In a randomized trial, 97 selected cases were divided into contrast group （ n = 48 ） and treatment group （ n = 49 ）. The clinical efficacy, the liver function and the amelioration of hepatic fibrosis biochemical indicator were observed. Results ：The indication of liver func-tion, B- ultrasound check and hepatic fibrosis biochemical indicator in treatment group had distinct amelioration after treatment（ P 〈 0. 01, P 〈 0.05 ）. The total clinical efficiency of treatment group（95.92% ）was more significant than that in the control group（75% ）（P 〈 0. 01 ）. The biochemical indicators of two group patients both had amelioration after treatment, and the treatment group was more significantly in amelioration with the serum levels of TBil, Alb, PVID, HA, IVC and LN（P 〈0. 01 ,P 〈0.05）. The rate of relapse of treatment group in 6th and 12th month（separately in 17.14% and 42.86% ）was more significant than that in the control group（ separately in 40.54% and 83.78% ） （P 〈0. 01 ）. Con clusion：Integrated Chinese medicine with western medicine obviously improves the symptoms and physical signs of the pa tients with cirrhosis, and raises clinical curative effect and the prostecdtive efficacy is better, accordingly improves the quality of life of the patients.
