寰枢椎脱位手术镁合金与钛合金TARP内固定系统在屈伸状态下的三维有限元分析 被引量:4

摘要 目的:对前路寰枢椎脱位的患者,应用三维有限元分析法对镁合金与钛合金材料TARP钢板固定,进行屈伸状态下对比分析,为临床提供理论依据。方法:选择一例寰枢椎脱位患者,进行CT扫描和三维重建,将数据导入有限元分析软件,对结果进行统计学分析。结果:表明上颈椎的有限元模型外形逼真,几何相似性好,共包含67846个节点,225368个单元。在模拟前屈时,寰枢椎在颈1/2关节突关节应力比较集中,其次是齿状突基底部。镁合金和钛合金固定材料有显著性差异(P<0.05)。结论:应用CT扫描获取上颈椎空间结构信息建立的上颈椎模型可用于生物力学实验,镁合金材料比钛合金材料临床固定效果更好,值得在临床进一步推广应用。 Objective: To discuss the application different of materials transoralpharyngeal atlantoaxial reduction plate (TARP) in the treatment of unreducible adantoaxial dislocation by using finite element analysis methods, and to provide theoretical support for its clinical application. Methods: A typical example ofatlantoaxial fracture dislocation patients, and CT thin titanium alloy and the clinical application of TARP system scan, and access to DICOM image data, import three-dimensional finite element analysis software, were simulated using the two kinds of titanium and magnesium alloy different materials preparation TARP reduction and internal fixation, Results: The results were statistically analyzed. Built in the shape of atlantoaxial dislocation reduction and internal fixation of the finite element model realistic, geometric similarity, a total of 67846 nodes, 225368 units; in the simulated head is located immediately, the stress concentration region were neck1/2 of the facet joints, followed by the fascia dentate basement; the two different materials of magnesium alloy and titanium alloy preparation of TAP, P system, the distribution of the corresponding mechanical there are significant differences (P〈0.05). Conclusion: CT scan was established to obtain structural information in atlantoaxial model can be used for biomechanical experiments; The use of existing titanium TARP treatment of atlantoaxial dislocation, and maintain the anterior arch of atlas, arch is complete, and fusion of the lateral mass indeed, you can better maintain the stability of atlantoaxial; atlantoaxial fusion, the facet joints of adjacent segments increased the force to accelerate the degeneration, should be closely followed up; The magnesium alloy materials for preparation of the TARP system better than titanium TARP in the stress concentration region lower and peak stressdistribution improved, needs further study.
