合并大型体肺侧支的肺动脉闭锁Ⅰ期单源化分期Fontan术的探讨

摘要 目的探讨合并大型体肺侧支(major aortopulmonary collaterals,MAPCAs)的肺动脉闭锁患儿通过分期手术干预后行全腔-肺静脉连接(Fontan)术的临床效果。方法 1例肺动脉闭锁、单心室合并MAPCAs患儿于出生后3个月Ⅰ期行单源化手术及改良Blalock分流术,12个月行Ⅱ期双向Gleen术,41个月时完成Fontan术,定期监测患儿肺动脉发育状况。结果通过前期的手术干预,肺动脉发育迅速,肺动脉指数持续增长,最终患儿良好耐受功能单心室化。结论早期单源化及分期手术干预最终能使合并MAPCAs的肺动脉闭锁患儿肺血管充分发育,满足Fontan手术要求。
