摘要
[目的]观察清热利湿通络汤治疗急性痛风性关节炎疗效。[方法]使用随机平行对照方法,将60例门诊患者按随机数字表方法随机分为两组,对照组30例布洛芬缓释胶囊300mg/次,2次/d;碳酸氢钠片1.5g/次,3次/d。治疗组30例自拟清热利湿通络汤(金银花30g,防己10g,牛膝15g,生薏苡仁30g,车前子20g,威灵仙15g,元胡、山慈菇、木瓜各12g,金钱草30g,黄柏12g,甘草、萆薢、郁金各10g)1剂/d,水煎分2次服,连续服用7d。观测临床症状、血尿酸、疼痛评分。7d为1疗程,连续治疗2疗程,随访4周,判定疗效。[结果]血尿酸、疼痛积分均有改善(P<0.01),治疗组优于对照组(P<0.01)。治疗组临床痊愈10例,显效16例,有效2例,无效2例,不良反应1例,总有效率93.30%。对照组痊愈8例,显效12例,有效4例,无效6例,不良反应9例,总有效率80.00%。治疗组疗效优于对照组(P<0.05)。[结论]清热利湿通络汤治疗急性痛风性关节炎可提高临床疗效,无明显不良反应。
[Objective]To observe the heat and dampness through contact soup treatment of acute gouty arthritis.[Method]Randomized controlled random number table method,60 outpatients were randomly divided into two groups,the control group of 30 patients Ibuprofen Sustained Release Capsules 300mg / 2 times / d;sodium bicarbonate tablets 1.5g / time,3 times / d.Treatment group,30 cases since the proposed heat and dampness Soup tongluo（honeysuckle 30g,the Menispermaceae 10g,Achyranthes 15g,the Health Yiyiren 30g,plantain seed 20g,15g clematis,corydalis,indica,papaya 12g,money grass 30g,Cork 12g,licorice,Dioscorea collettii,turmeric 10g） Ji/d,decoction 2 times service continuous taking 7d.Observation of clinical symptoms,serum uric acid,the pain score.Seven days for a course of treatment,continuous treatment course,followed up for 4 weeks to determine efficacy.[Results]The serum uric acid,pain scores were improved（P0.05）,the treatment group than the control group（P0.05）.Treatment group,10 cases were cured,16 cases,2 cases,invalid 2 cases,1 cases of adverse reactions,the total efficiency of 93.30%.Control group,8 cases were cured,12 cases markedly effective in 4 cases,6 cases,nine cases of adverse reactions,the total effective rate of 80.00%.Treatment group than the control group（P0.05）.[Conclusion]Heat and dampness tongluo of soup treatment of acute gouty arthritis can improve clinical outcomes,no significant adverse reactions.
