摘要
目的观察双白止痒洗剂治疗慢性手湿疹的疗效,并探讨其作用机制。方法共60例慢性手湿疹患者纳入观察,以随机数字表法随机分为观察组和对照组。观察其症状转归情况,治疗后随访3个月,比较2组患者第1次复发的时间。治疗前后采用流式细胞仪检测2组患者的CD3、CD4、CD8淋巴细胞。结果观察组治愈13例,显效6例,好转6例,总有效率为83.3%;对照组治愈5例,显效5例,好转6例,总有效率为53.3%。经Ridit分析,P=0.007 5,P<0.01,提示观察组疗效优于对照组。治疗后检测四种T淋巴细胞亚群细胞可见,治疗组的CD3+、CD4+均值较治疗前相比有明显升高,但对照组的CD3+、CD4+升高不明显,治疗后2组CD3+、CD4+相比其差异有统计学意义。结论双白止痒洗剂能迅速消除患者的症状,并能明显延长或阻止湿疹的复发,远期效果好,其作用机理与调节免疫功能,抑制T淋巴细胞的活化有关。
Objective To observe the efficacy of Shuangbai Zhiyang lotion on chronic hand eczema,and to explore its mechanism.Methods A total of 60 patients with chronic hand eczema in the observation were randomly divided into two groups with a random number table,the observation group and the control group.The outcome of their symptoms were observed in follow-up three months,and then compared the first relapse time between these two groups.CD3,CD4 and CD8 lymphocytes of patients were detected by flow cytometry before and after treatment.Results 13 cases in the observation group had been cured after treatment,and total effective rate was 83.3%.And five cases in the control group had been cured,and total effective rate was 53.3%.By Ridit analysis,curative effect of observation group was better than that of control group（P=0.007 5,P0.01）.After treatment,four kinds of T-lymphocyte subsets could be detected.Compared to the mean before treatment,CD3＋ and CD4＋ was significantly increased in the treatment group,but it was not increased obviously in the control group.After treatment,the differences of CD3＋ and CD4＋ were significant.Conclusion Shuangbai Zhiyang lotion can eliminate symptoms quickly,and it can prolong or prevent the recurrence of eczema significantly.Long-term effect is good.Its mechanism may be related to the effect on immune function and inhibiting T lymphocyte activation.
