二维及彩色多普勒超声对甲状腺微小乳头状癌的诊断价值 被引量:4

摘要 目的探讨二维与彩色多普勒超声对直径<10mm甲状腺微小乳头状癌(papillary thyroid microcarcinoma,PTC)的诊断价值。方法回顾性总结分析经手术病理证实的376例单、多发结节性病灶中的单发86例PTC超声与相关临床资料,并将其二维及彩色多普勒声像特征与病理结果对照。结果 86例PTC中除5例超声诊断不符合外,81例诊断正确,占94.1%。PTC的主要声像特征为:低或极低回声结节、A/T比值≥1、微钙化、不规则毛刺状边缘及少数结节血流相对丰富、颈部淋巴结肿大。结论甲状腺PTC的二维及彩色多普勒声像图具有特异性,本检查方法具有重要临床价值。 Objective To explore the value of two-dimensional and color Dopple ultrasound in diagnosis of papillar thy- roid microcarcinoma (PTC) which was less than 10 mm in its diamete. Methods Ultrasonic features and relative clinical data of 86 PTC cases included in 376 cases with solitary or multiple nodules were retrospectively analyzed. All of those ca- ses were confirmed by operation and pathology. Results Among those 86 cases, 81cases were diagnosed correctly by ul- trasound and the diagnose accordance rate was 94.1%. The majority masses showed A/T≥1, hypoechoic or very hypoe- choic nodules with internal microcalcification and irregular margin. A few cases showed hypervascularity in CDFI images and cervical lymph nodula metastasis. Conclusion Two-dimensional and color Dopple ultrasound show great value in the diagnosis of thyroid papillary microcarcinoma.
