摘要
目的分析医嘱的合理性,促进临床合理用药。方法收集和分析我院两年来静脉用药调配中心医嘱审方中不合理用药的实例。结果不合理用药主要为配伍禁忌、溶媒的不合理选择、药物超剂量、不合理给药方式等问题。结论对静脉用药不合理医嘱的分析和干预,提高了静脉药物治疗的合理性、安全性及有效性,为临床合理用药提供参考。
Objective To promote rational drug use by analyzing the rationality of prescription.Methods The cases of irrational drug usage in the past two years were collected and analyzed in our pharmacy intravenous admixture services（PIVAS）.Results There were problems of irrational drug use such as drug incompatibility,irrational choice of solvents,exceeded dosage,and unreasonable route of administration.Conclusion The rationality,safety and efficacy of intravenous therapy were improved through the retrospective study and intervention of irrational prescription which provided references for clinical rational drug use.
