摘要
目的探讨布鲁菌病并发中枢神经系统感染的护理和预防。方法收集首都医科大学附属北京地坛医院收治的布鲁菌病例,分析其流行病学特征及临床表现,对伴有中枢神经系统感染的重症病例的护理情况进行回顾性分析。结果 2001年9月~2011年10月本院共收治布鲁菌病患者56例,其中仅2011年收治该病患者18例(32.1%)。入组的56例患者中非农牧民24例(42.9%);伴中枢神经系统感染者9例,其中男性7例,女性2例,年龄3~58岁,平均年龄29.25岁。9例患者均伴有头痛,4例伴有呕吐,9例患者均伴有脑脊液压力增高、细胞计数增加(单核细胞为主)和蛋白质含量增高。9例患者中2例痊愈,6例好转,1例自动出院,住院2~57d,平均住院时间28d。结论严密、细致地观察患者的病情变化,及时给予相应的治疗和护理,可降低布鲁菌病并发症的发生率,提高治愈率。同时加强对患者的健康宣教,对预防及降低该病的发生率亦具有重要的意义。
Objective To investigate the nursing care and prophylaxis of patients with brucellosis complicated with central nervous system infection. Methods Cases of brucellosis from September 2001 to October 2011 admitted to Beijing Ditan Hospital, Captital Medical University were enrolled, and characteristic of epidemiology and the clinical manifestations were analyzed. The problem of nursing care on the severe cases of brucellosis complicated with central nervous system infection was also analyzed. Results There were 56 patients with brucellosis treated in our hospital, 18 cases of whom were enrolled in 2011. Total of 24 patients were neither peasants nor herdsmen, who took up 42.9% of all. There were 9 patients complicated with central nervous system infection, 7 of whom were male and 2 female, with an average age as 29.25 years (3-58 years old). All the 9 patients had headache, 4 of whom vomitted, with increased cerebrospinal fluid, cerebrospinal (mainly monocyte) and protein. Among the 9 patients, 2 were cured, 6 improved, and 1 gived up. They were treated in hospital for 2 to 57 days, with an average time as 28 days. Conclusions With careful observation and timely treatment and care, the incidence of brucellosis complications could be reduced and the cure rate could be increased. Meanwhile, strengthening education on health would play an important role to prevent and reduce the incidence of the disease.
