飞秒激光和角膜板层刀制瓣准分子激光原位角膜磨镶术的临床效果 被引量:4

The clinical observation of laser in situ keratomileusis with corneal flap created by femtosecond laser and microkeratome

摘要 目的比较应用飞秒激光和角膜板层刀制瓣准分子激光原位角膜磨镶术(LASIK)的临床效果。方法回顾性分析400例(800眼)近视手术,根据制作角膜瓣方法不同分为两组,每组均为200例(400眼),组1为飞秒激光(Intralase FS60 kHz)制作角膜瓣,组2为Moria M2 90号板层刀制瓣。两组均在Wavescan波前引导下VISXStars。准分子激光进行基质层切削。术后3个月、6个月行视力、屈光状态、波前像差等检查。结果术后3个月、6个月两组视力和屈光度比较差异无统计学意义(P〉0.05)。术后3个月,组2总高阶像差、彗差、球差高于组1,两组差异有统计学意义(P〈0.05),三叶草像差比较差异无统计学意义(P〉0.05)。术后6个月,组2总高阶像差、彗差、球差、三叶草像差均高于组1,两组差异有统计学意义(P〈0.05)。结论应用飞秒激光和角膜板层刀制瓣LASIK术是矫正近视安全有效的方法,在提高视觉质量和安全性方面飞秒激光优于角膜板层刀制瓣LASIK术。 Objective To compare the clinical results of laser in situ keratomileusis (LASIK) flap made with femtosecond laser and with microkeratome. Methods 400 patients (800 eyes) with myopia re- ceived operation of LASIK. The patients were divided into two groups according to the methods of making corneal flap. The patients of group one were assigned to receiving LASIK with corneal flap creation by in- tralase femtosecond laser(400 eyes of 200 patients). The patients of group two with corneal flap creation by microkeratome. Both two groups receiving eximer laser ablation with VISX Star S4. Uncorrected visual acuity , refractive diopter, aberrations were checked and recorded at 3 month and 6 month postoperatively. Results The refractive diopter and uncorrected visual acuity between the two groups had no significantly difference at the 3 and 6 month postoperation (P 〉 0.05 ). At the 3 month postoperation, root of mean square of higher aberration, coma aberration and sphere aberration in group 2 were higher than that in group 1 (P 〈 0.05 ), trefoil aberration had no significant difference between the two groups( P 〉 0.05 ). At the 6 month postopera- tion, root of mean square of higher aberration, coma aberration, sphere aberration and trefoil aberration in group 2 were higher than that in group 1 ( P 〈 0.05 ). Conclusion LASIK corneal flap with femtosecond la- ser and microkeratome are both safe and effective for the correction of myopia, but femtosecond laser is much better than LASIK flap made by microkeratome in improving visual performance and security.
