右美托咪啶预防瑞芬太尼麻醉术后痛觉过敏的效果 被引量:12

摘要 目的:观察右美托咪啶预防瑞芬太尼麻醉患者术后痛觉过敏的效果。方法:选择择期行胃肠手术患者80例,ASAⅠ或Ⅱ级,年龄35~64岁,体重50~86kg。随机分成4组:对照组(C),D1、D2、D3组,每组20例。静脉依次注射力月西2mg,舒芬太尼0.4g/kg,异丙酚1~2mg/kg,顺阿曲库铵0.15mg/kg麻醉诱导。TCI瑞芬太尼(血浆靶浓度1.5~3ng/ml),复合吸入2%~4%七氟醚,静脉输注顺苯阿曲库铵1.2ug.kg-1.min-1维持麻醉。术毕前30min对照组静脉注射生理盐水10ml,D1、D2、D3组静脉分别输注右美托咪啶0.2、0.4和0.6g/kg(生理盐水稀释至10ml),记录患者拔管时间、术后2h及24hVAS评分和舒芬太尼用量,记录恶心呕吐和烦躁的发生情况。结果:与对照组相比,D2、D3组2h内的舒芬太尼用量减少,VAS和躁动评分降低(P<0.05);术后2~24h内VAS评分和舒芬太尼用量差异无统计学意义(P>0.05);D3组拔管时间延长(P<0.05)。结论:右美托咪定0.4g/kg可有效预防瑞芬太尼诱发的痛觉过敏,而不影响苏醒拔管时间。
