摘要
目的了解本院门诊药房退药原因,以研究对策,减少退药发生,保证患者用药安全。方法回顾分析2011年1～12月门诊药房退药记录,按退药科室、退药原因、退药种类以及患者年龄进行分类统计,计算构成比,并进一步分析原因,提出解决对策。结果共统计退药事件474起,涉及药品559种,其中西药房退药事件316起(66.7%),共退药品377种;中药房退药事件158起(33.3%),涉及药品182种。退药最多的3个科室为儿科、心内科和神经内科,分别为131例(23.43%)、99例(17.71%)和85例(15.21%),3个科室退药占门诊退药的56.35%。主要退药原因为用药不良反应(34.60%),转为住院治疗(11.39%)以及医保问题(11.60%)。医院针对此修订完善了退药制度。结论门诊退药是个复杂的问题,需在确保医院药品质量的前提下完善各项制度,并提高医务工作者的诊疗水平,努力避免退药现象发生。
Objective To find the reasons of drug repercussion in out-patient pharmacy,then to find the countermeasure and to promote rational use of drugs.Methods A total of 474 pieces of drug repercussion records were collected from January to December 2011,drug repercussion records were analized statistically in respect of the deparment of drug repercus sion,reasons of drug repercussion,types of returned drugs and the age of patients.Results There were 316 pieces（66.7%） of drug repercussion records in out-patient pharmacy,and 158 pieces（33.3%）in traditional Chinese medicine pharmacy.The departments of drug repercussion were mainly pediatric department（131 pieces,23.43%）,cardiology department（99 pieces,17.71%）,and neurology department（85 pieces,15.21%）.The main causes of out-patient drug repercussion were drug adverse reaction or event（34.60%）,turns to be hospitalization（11.39%）and something about medical insurance（11.60%）.It was urgent to establish drug repercussion system and the system of reward and punishrnent.Conclusion Drug repercussion is a complicated issue,it requires the improvement of relevant system and skill of medical staff,and avoid occurrence of drug repercussion.
