动力髋螺钉内固定治疗股骨粗隆间骨折术后股骨头切割发生原因及对策 被引量：2

摘要 目的探讨动力髋螺钉(DHS)内固定治疗股骨粗隆间骨折术后股骨头切割发生原因及对策。方法 28例股骨粗隆间骨折行闭合复位DHS内固定手术治疗,术后发生股骨头切割2例,对其临床资料进行分析。结果本组随访时间为12～36个月,中位时间24个月;2例患者发生穿出、切割股骨头伴髋内翻畸形,其中1例(头钉位于股骨头颈内上方)术后13个月时拆除内固定,骨折畸形愈合;另1例(过早负重)发生股骨头坏死,18个月后再次行全髋关节置换手术。结论对DHS治疗股骨粗隆间骨折应正确把握手术适应证、规范手术操作及正确的术后指导,可有效防治术后股骨头切割的发生。
