摘要
目的分析癌性贫血的危险因素,探讨癌性贫血对恶性肿瘤患者生存质量(QOL)的影响。方法选取2009年1月～2010年12月在乐山市人民医院肿瘤科住院治疗的癌性贫血患者80例,选取同期住院的非癌性贫血的恶性肿瘤患者80例为对照,回顾性分析性别、年龄、经济状况、肿瘤类别、临床分期、KPS评分、血清清蛋白水平、放疗和化疗等因素与癌性贫血的相关性。采用健康状况调查问卷简表SF-36对两组患者进行调查,分析癌性贫血对QOL的影响。结果癌性贫血与年龄、临床分期、KPS评分、血清清蛋白水平、放疗和化疗密切相关,P<0.05;与性别、经济状况和肿瘤类别无密切关系,P>0.05。癌性贫血组生理功能(PF)、生理职能(RP)、总体健康(GH)和精力(VT)4个维度分值显著低于对照组,P<0.05;躯体疼痛(BP)、社会功能(SF)、情感职能(RE)和心理健康(MH)4个维度分值在两组间差异无统计学意义,P>0.05。结论高龄、晚期、低血清清蛋白水平、接受放疗和化疗等因素是癌性贫血的独立危险因素。癌性贫血会显著降低恶性肿瘤患者的QOL。
Objective To explore risk factors of malignant tumor anemia and to investigate the influence of anemia on quality of life（QOL） of patients with malignant tumor.Methods Clinical data of 80 cases of malignant tumor anemia admitted to Leshan People＇s Hospital from Jan.2009 to Dec.2010 were retrospectively analyzed and compared with those of 80 cases of malignant tumor without anemia admitted to the same hospital in the same period to explicit the impact of gender,age,financial situation,classification of tumor,clinical stages,KPS scores,serum albumin levels,radiotherapy and chemotherapy on anemia.All patients were assessed with Short Form 36 Health Survey Questionnaire（SF-36） to clarify the impact of anemia on QOL in patients with malignant tumor.Results Malignant tumor anemia was correlate with age,clinical stages,KPS scores,serum albumin levels,radiotherapy and chemotherapy,P0.05.No correlation was found between anemia and gender,financial situation or classification of tumor,P0.05.The scores of physical function（PF）,role physical（RP）,general health（GH） and vitality（VT） were significant lower in anemia group than those in control group,P0.05,while no significant difference were found in scores of bodily pain（BP）,social function（SF）,role emotional（RE） and mental health（MH） when compared between the two groups,P0.05.Conclusion Advanced age,later period,low serum albumin level,radiotherapy and chemotherapy were independent risk factors of malignant tumor anemia.Anemia would marked brought down QOL of malignant tumor patients.
