外源性甲状腺素对心内直视术围术期心肌能量代谢酶的影响 被引量:5

Effects of thyroid hormone on enzyme of myocardial energy metabolism during open heart surgery

摘要 目的 研究外源性甲状腺素 (TH)对心内直视术围术期心肌能量代谢的影响。方法将二尖瓣置换术 (MVR)患者随机分为用药组和对照组 ,术前 4~ 7d用药组服甲状腺片 ,对照组服安慰剂。检测术前、中、后心肌乳酸脱氢酶 (LDH)、细胞色素氧化酶 (CCO)、琥珀酸脱氢酶 (SDH)、钙ATP酶 (Ca2 + ATPase)活性的变化。结果 用药组SDH、CCO、Ca2 + ATPase的活性比对照组增高(P <0 .0 0 1) ,两组LDH差异无显著性 (P >0 .0 5 )。对照组 1例死亡 ,术前、术中、术后SDH、CCO、LDH、Ca2 + ATPase的活性显著降低。结论 术前补充TH ,可提高心肌的有氧代谢、能量合成 ,促进心功能。 Objective\ To study the effects of thyroid hormone (TH) on the myocyte energetic metabolism during open heart surgery. Methods\ The activities of human myocyte lactate dehydrogenase (LDH), cytochrome oxidase (CCO), succinic acide dehydrogenasel(SDH), Ca 2+ ATPase during ischemia/reperfusion were determined by using the histochemical method. Results\ The activities of CCO, SDH, Ca 2+ ATPase in the treated group were higher than in the control group. No significant difference in LDH was found between the two groups. The activities of SDH, CCO, LDH and Ca 2+ ATPase declined in the death case in the control group. Conclusion\ Pre treatment with TH 4~7 days before operation could improve the myocyte aerobic metabolism, energetic synthesis, and the heart function.
