摘要
目的 :探讨胎膜早破孕妇直肠内应用米索前列醇促宫颈成熟和引产的临床效果及可行性。方法 :选择足月妊娠、胎膜早破、宫颈 Bishop评分≤ 6分的孕妇共 85例 ,分成 组 44人 ; 组 41人 ,分别口服和直肠内应用米索前列醇 5 0μg,每 3h一次。观察用药到临产的时间、产程进展、分娩结局及副反应的发生等情况。结果 :首次用药到规律宫缩的时间 组明显短于 组 (P<0 .0 1)。宫缩过强致胎儿窘迫的例数 组明显多于 组。其余指标两组无显著差异。结论 :与口服用药相比 ,直肠内应用米索前列醇具有药效稳定、持续时间长、副反应少的优越性。对胎膜早破的孕妇可用其代替阴道局部用药。
Objective:To determine the effectiveness and feasibility of misoprostol used in intra rectum for the cervical ripening and labor induction of women with pre labor rupture of membrance at term.Methods:85 women with term pregnancy;pre labor rupture of membrane and Bishop score≤6 were assigned randomly to group Ⅰ(44 patients)and groupⅡ(41 patients).They were given misoprostol 50 μg,every 3 hours orally or intra rectum respectively.Then we compare the differences of 1. The interval from the first dose to the onset of labor;2. the progress of labor;3.the safety of the mothers,fetuses and neonates;4.the adverse effects of the drug between the two groups.Results:The time from the first dose to the onset of labor in group Ⅰ (176.3±74.8min)was shorter than group Ⅱ(206.64±81.5min) P <0.01.More fetal distresses caused were by hyperstimulation were found in group Ⅰ than in group Ⅱ.Other indexes observed had no statistical differences between the two groups.Conclusion:Compared to misoprostol orally,misoprostol used in intra rectum was more stable in clinicl effectiveness and had less adverse effects,similar to used intra vaginally.So misoprostol administered intra rectum can replace that it was used intra vaginally for the term pregnant women with pre labor rupture of membrane.
