摘要
目的总结血小板减少症的诊断、鉴别诊断和治疗经验。方法分析86例血小板减少症患者的临床表现、实验室检查结果等临床资料。结果 86例患者中有继发原因引起血小板减少者14例,原发72例,合并其他疾病11例。一线肾上腺皮质激素治疗有效45例,脾切除术3例有效;2例治疗无效。结论本组病例选择外周血、骨髓穿刺检查、肝肾功、血小板相关抗体、甲状腺功能、系统性红斑狼疮抗体、乙肝丙肝检查、腹部B超等检查,在鉴别诊断是必要的。经典治疗存在无效病例。促血小板生成素、促血小板生成素受体激动剂治疗将成为新的治疗方法。
Objective We summarized 86 cases of thrombocytopenia in the diagnosis, differential diagnosis and treatment experience. Methods 86 patients with thrombocytopenia in patients with clinical manifestations, laboratory ex- amination etc were analyzed. Results In 86 patients, there were 14 cases with secondary causes of thrombocytopenia and 72 cases with primary. Combined with other diseases in 11 cases, there were 45 cases had effective treatment in the first line of adrenal cortical hormone therapy. There were 3 cases had effective treatment in the aspect of splenectomy. 2 cases had ineffective treatment. Conclusion The group of cases has selected peripheral blood, bone marrow biopsy, liver and kidney function, platelet associated antibody, thyroid function, systemic lupus erythematosus, hepatitis B, hepatitis C antibody examination, abdominal B ultrasound examination etc. In the differential diagnosis is necessary. Invalid cases are existed in the classic treatment. The treatment of thrombopoietin receptor agonist will open up the new treatment.
