彩色多普勒超声引导下乳腺良性肿块微创切除的临床应用 被引量:6

Clinical application of minimally invasive surgery in resecting benign breast lumps under the guidance of color Doppler ultrasound

摘要 目的探讨彩色多普勒超声引导下微创切除术在乳腺良性肿块中的应用价值和注意要点。方法在彩色多普勒超声引导下对78例患者的125个乳腺肿块行微创切除并活检,记录术后情况,并与传统切除手术病例进行随访,比较2者的差异。结果微创切除手术患者的125个乳腺肿块可完全切除,所切除的肿块病理类型如下:纤维腺瘤45个(36.0%),导管内乳头状瘤4个(3.2%),囊肿5个(4.0%),乳腺纤维增生症20个(16.0%),乳腺囊性增生26个(20.8%),乳腺病25个(20.0%)。与传统切除术比较,彩色多普勒超声引导下微创切除术组患者平均住院天数、术后感染及术后疼痛减少,随访满意度显著增高。结论彩色多普勒超声引导下切除乳腺良性肿块具有操作简单、创伤小、无明显疤痕、并发症少及患者随访满意度高的优点,值得推广应用。 Objective To investigate the value of minimally invasive surgery in resecting be- nign breast lumps under the guidance of color Doppler ultrasound. Methods The excision of 125 le- sion in 78 patients was performed with Maznmotome system under the guidance of ultrasound be- tween September 2008 and September 2010. The complications and degree of satisfaction after re- section were recorded and compared with patients experiencing traditional surgical removal of breast lesions. Results Ultrasound could clearly show the process of resecting benign breast lumps. A total of 125 breast lesions in 78 patients were completely resected. The pathological types of 125 lesions were as follows: fibroadenoma in 45 (36 % ), intraductal papilloma in 4 (3.2 % ), cyst in 5 (4 % ), breast fibrous hyperplasia in 20(16% ), cystic hyperplasia in 26(20.8% ), and other breast disease in 25 (20 % ). Compared with traditional surgical removal, minimally invasive resection with Mam- motome system under the guidance of ultrasound could reduce hospitalization and complications after surgery with a high degree of satisfaction during follow-up. Conclusion Complete removal of clini- cally benign breast lesions with the ultrasound-guided Mammotome system is easily achieved and it has fewer complications with a high degree of satisfaction during follow-up. This method is worthy of being widely applied.
