摘要
目的探讨控制性低中心静脉压技术(LCVP)在肝叶切除术中对凝血功能以及血小板计数变化的影响。方法本研究将普外科2010年3月～2012年1月收治的30例行肝叶切除术的患者按照随机对照原则分为两组,每组15例,其中术中维持正常的中心静脉压组(C组)将中心静脉压维持在正常值的8～12cmH2O,术中控制性低中心静脉压组(L组)将中心静脉压减低到2～4cmH2O,观察两组凝血指标。结果 C组中手术期间较手术前的血小板减少,而凝血酶时间(TT)、凝血酶原时间(PT)以及活化部分凝血活酶时间(APTT)均较术前延长;手术期间L组与C组比较,L组的PLT较C组的明显升高,而TT、PT以及APTT均较C组明显减低,差异有统计学意义(P<0.05)。结论控制性低中心静脉压技术在肝脏手术中应用,尤其对凝血以及血小板的影响较为明显。
Objective To investigate the effectiveness of low central venous pressure （LCVP） technique in liver lobe resection operation on coagulation function and platelet changes. Methods 30 cases of hepatectomy patients in our hospital from March 2010 to January 2012 were chosen, and were divide into two groups, each group 15 cases, in which the normal central venous pressure group （ group C） will be the central venous pressure was maintained in the normal value of 8-12mmHg, low central venous pressure group （ group L） will be the central venous pressure is reduced to4-5mmHg, observe two groups of coagulation index. Results In C group during operation than before operation the thrombocytopenia, while TT, PT and APTT were prolonged preoperative; operation period in L group compared with C group, L group PLT than in C group increased significantly, while TT, PT and APTT were lower than group C significantly decreased, there was significant difference （P〈0.05）. Conclusion Controlled low central venous pressure in liver operation application, especially on coagulation and platelet effect more obvious.
