瑞芬太尼在七氟醚快诱导无肌松气管插管期间防止高血压反应的最佳效应室浓度 被引量:9

摘要 目的确定吸入七氟醚诱导无肌松气管插管期间为了防止高血压反应时瑞芬太尼的最佳效应室浓度。方法 ASAⅠ或Ⅱ级,年龄18~55岁的患者80例,按入室顺序随机分为四组,吸入3%七氟醚麻醉诱导,之后每组患者瑞芬太尼效应室靶浓度(Ce)分别设定为3、4、5或6ng/ml(Ce3、Ce4、Ce5、Ce6组),注毕90s后气管插管。采用概率分析法计算使95%的患者气管插管时不出现高血压反应的效应室靶浓度。结果 95%患者插管时无高血压反应的瑞芬太尼效应室浓度为5.36ng/ml(95%CI:4.73~7.07ng/ml)。结论 3%七氟醚诱导无肌松气管插管期间无高血压反应的瑞芬太尼最佳效应室浓度为5.36ng/ml。 Objective The aim of the present study was to determine the effect-site concentration of remifentanil to prevent hypertension during tracheal intubation under sevoflurane anesthesia. Methods Eighty patients, aged 18-55 years old, ASA I or 11, were randomly divided into four groups. All patients received 3% sevoflurane anesthesia induction followed by various effecvside concentrations of remifentanil[group Ce3 (3 ng/ml), Ce4 (4 ng/ml), Ce5 (5 ng/ml), Ce6 (6 ng/ml)]. Patients were intubated 90 seconds later. A probit analysis was used to calculate remifentanil effect-site concentrations required to prevent hypertension in 95% of all the patients. Results The effect-site concentration of remifentanil to prevent hypertension in 95% of the patients was 5.36 ng/ml (95% CI, 4. 73-7.07 ng/ml). Conclusion The effect-site concentration of remifentanil to inhibit hemodynamic response is 5. 36 ng/ml during 3% sevoflurane anesthesia without muscle relaxant.
