摘要
目的探讨牙周-牙髓联合病变(逆行性牙髓炎)的治疗方法和临床体会。方法选择19颗逆行性牙髓炎患牙,做常规根管治疗、牙周基础治疗、药物治疗和牙周支持治疗并观察其疗效。结果 19颗患牙经过半年疗效观察成功率是100%,经过1～3年疗效观察成功率是73.7%。5颗患牙治疗半年后,不做定期复查和牙周支持治疗病情复发,治疗失败。结论根管治疗、牙周基础治疗、药物治疗和牙周支持治疗相结合,是临床上治疗逆行性牙髓炎有效的方法。如果不做定期复查和牙周支持治疗病情回到治疗前的状态,导致治疗失败。中度牙周病比重度牙周病治疗效果好。
Objective To discuss clinical experiences in the treatment of combined periodontics-endodontic lesions（retrograde pulpitis）.Methods We chose 19 teeth that suffered retrograde pulpitis.The effects were observed after routine root canal therapy,initial periodontal treatment,drug treatment and supportive periodontal therapy（SPT）.Results the curing rate（100%） was higher in the sixth month after treatment than that（73.7%） after 1-3 years.Relapse occurred in five cases after half a year,because both regular reexamination and supportive periodontal therapy were not carried out.Conclusions combining therapy is effective for retrograde pulpitis,in which routine root canal therapy,initial periodontal treatment,drug treatment and supportive periodontal therapy（SPT） are integrated.Regular reexamination and supportive periodontal therapy are necessary.Otherwise,the condition will return to the state before treatment and therapies fail finally.Therapeutic effect on moderate periodontosis is better than that on specific weight periodontosis.
