彩超引导下经皮肾钬激光碎石术治疗肾旋转不良结石的探讨 被引量:1

Color Doppler Ultrasound -guided Percutaneous Nephrostomy and Holmium Laser Lithotripsy in Treatment of Renal Malrotation with Lithiasis

摘要 目的探讨彩色多普勒超声引导下经皮肾钬激光碎石术治疗肾旋转不良结石的疗效与安全性。方法回顾性分析17例彩色多普勒超声引导下经皮肾钬激光碎石术治疗肾旋转不良结石病例的临床资料。其中腹中线位型10例,腹侧位型5例,侧向型2例,无过度旋转型(即背侧型)。结石最大径19~43(25±6.1)mm。结果手术均获成功,无术中转开放性手术。手术时间55~159(平均93)min。出血量30~117(平均41)ml。一次经皮肾镜取石术结石取净率为82.35%,所有病例未见出现严重的并发症。结论彩超引导下经皮‘肾钬激光碎石术治疗是一种治疗。肾旋转不良结石安全、有效的方法。 Objective To investigate the safety and feasibility of color doppler ultrasound - guided percutaneous nephrolithotomy(PCNL) with holmium laser lithotripsy in the treatment of renal malrotation with lithiasis. Methods Seventeen cases( From March 2010 to April 2012)with renal malrotation and lithiasis were treated with percutaneous nephrolithotomy(PCNL) with holmium laser lithotripsy guided by color doppler ultrasound and were retrospective ana- lyzed, among which were 10 kidneys with imcomplete rotation,5 kidneys with absent rotation,2 kidneys with derotation and no overrotation. The biggest calculi diameter :19 to 43 mm and the average size of calculi was 25 ±6.1 mm. Re- suits The all operations were successful without transferring to open surgery,operation time was 55 to 159 min with an average of 93 min. The calculi were cleared with one stage PCNL in 14 cases, two stage PCNL in 3 cases and no se- rious complications appeared in all cases. Conclusion Color dopplar ultrasound guidance with percutaneous renal holmium laser lithotripsy therapy is a safe and effective treatment for renal malrotation with lithiasis.
