摘要
目的探讨锁定接骨板联合同种异体骨在四肢长骨良性骨肿瘤病理骨折中的修复作用与重建方法。方法 32例四肢长骨良性骨肿瘤伴有病理性骨折患者,术中刮除病灶后采用高速磨钻打磨腔壁,骨缺损处植入同种异体骨,锁定接骨板桥接固定骨折断端。结果术后随访10～42个月,平均27.6个月。27例病变愈合良好,骨缺损修复,骨折愈合,肿瘤没有复发。32例中复发3例,再骨折2例,均再次病灶清除植骨内固定治疗后痊愈。结论锁定接骨板联合同种异体骨充填治疗长骨良性骨肿瘤伴发病理性骨折安全、有效。术中彻底刮除病灶,高速磨钻打磨肿瘤壁,打压植骨,同时选用合适的锁定接骨板是提高临床疗效的有效方法。
Objective To study the effects of locked plate fixation and allograft bone transplantation in the treatment of benign bone tumor with pathologic fracture.Methods Thirty-two patients with benign bone tumor associated with pathologic fracture in extremity long bone were treated.After curettage of lesions,high-speed burr grinding and allograft bone filling,the fracture was stabilized by a locked plate.Results The mean duration of follow-up time was 27.6 months.Twenty-seven cases of lesions healed with no recurrence of tumor.There were 3 cases of local recurrence and 2 cases of refracture at the follow-up examination,all of them were recovered by curettage combined with bone transplantation and internal fixation again.Conclusion For patients with long bone pathologic fracture due to benign bone tumor,locked plate fixation and allograft bone filling is a safe and effective way to restore the limb function,and to improve the clinical efficacy through curettage of the lesion,high-speed burr grinding,punning bone transplantation and appropriate choice of locked plate.
