慢性心力衰竭患者尿液NGAL水平与肾功能恶化的关系 被引量:2

Relationship between urinary neutrophil gelatinase-associated lipocalin and worsening renal function in patients with chronic heart failure

摘要 目的探讨慢性心力衰竭患者尿液中性粒细胞明胶酶相关脂质运载蛋白(neutrophil gelati-nase-associated lipocalin,NGAL)水平与患者发生肾功能恶化的关系。方法慢性心力衰竭的患者共38例,采集入院和3天时尿液标本,用ELISA法测定尿液NGAL水平。根据患者肾功能是否恶化,将患者分为肾功能恶化组和肾功能未恶化组,比较两组间入院和3天时尿液NGAL水平。结果 38例患者中有11例患者(28.9%)发生了肾功能恶化。肾功能恶化组患者入院和3天时尿液NGAL水平分别高于肾功能未恶化组患者(P<0.01);且肾功能恶化组患者入院时尿液NGAL水平明显低于3天时尿液中NGAL水平(P<0.01)。肾功能恶化组患者入院和3天时尿液NGAL水平与血肌酐差值(3天和入院时)之间呈正相关。结论尿液NGAL水平升高可能作为慢性心力衰竭患者肾功能恶化的早期预警信号。 Objective To assess the relationship bewteen urinary neutrophil gelatinase-associated lipocalin(NGAL) and worsening renal function(WRF) in patients with chronic heart failure(CHF).Methods Data were collected on 38 patients who were hospitalized with CHF.Urinary samples were collected at admission and at day 3.The levels of urinary NGAL were measured by ELISA.The patients were divided into two groups: patients with WRF and without WRF,and the levels of NGAL at admission and at day 3 were compared between the two groups.Results There were 11 patients(28.9%) developed WRF among all the 38 patients.Compared with the patients without WRF,the levels of urinary NGAL were increased at admission and at day 3 in patients with WRF(P0.01);The levels of urinary NGAL at admission were obviously lower than that at day 3(P0.01) in patients with WRF.The levels of urinary NGAL in patients with WRF at admission were positively correlated with the difference values of serum creatinine(between the levels at day 3 and at admission),and the same as the level of urinary NGAL at day 3.Conclusion Monitoring of urinary NGAL levels may be used to predict WRF in patients with CHF.
