摘要
目的：探讨胃恶性肿瘤的ＭＲＩ特点及其诊断价值。材料和方法：回顾性分析３２例经病理证实胃恶性肿瘤的ＭＲＩ资料，其中胃腺癌２５例，胃淋巴瘤３例，胃平滑肌肉瘤３例，结肠癌侵犯胃壁局部１例。另有正常对照组１０例。所有病例均肌注盐酸消族山莨菪碱 １０～ ２０ｍｇ １０～ １５ｍｉｎ后口服 ０．１％钆喷酸葡胺 ８００～ １０００ｍｌ再行扫描。使用Ｐｈｉｌｉｐｓ ＡＣＳ－ＮＴ１５型１．５Ｔ磁共振成像仪，图像采集应用自旋回波序列，Ｔ１和Ｔ２加权像，部分加扫脂肪抑制和质子密度像。结果：对照组中胃壁及周围结构都清晰显示。 Ｍ ＲＩ能准确地显示胃肿瘤的大小、形态、侵及范围、远处转移，对胃恶性肿瘤浆膜外浸润也能及早发现。结论：ＭＲＩ对胃恶性肿瘤的诊断与分期有重要价值。
Purpose: To evaluate the characteristic MR findings and its diagnostic value of gastric malignant tumor. Materials and methods: MR imaging was performed in 32 patients with gastric malignant tumor confirmed by pathology , including 25 cases of gastric adenocarcinoma, 3 cases of gastric lymphoma, 3 cases of gastric leiomyosarcomas , and 1 case of colonic cancer with gastric wall invasion. 10 normal cases were for control group. Before scanning, 800 - 1000ml 0. 1 % solution of Magvist was taken alter 10 ~ 15 minutes of intramuscular injection of 10 ~ 20mg 654 - 2. T1 and T2 WI were obtained with spin - echo sequences. Proton density image and T1 and T2 STIR were also conducted in some cases. Results: The gastric wall and its adjacent structures could be shown clearly in the control group. All lesions, not only the gastric tumors themselves, but also the extraserosal infiltration and distant metastases were delineated conspicuously with both T1 WI and T2 WI. Conclusion: MRI could provide valuable information in the diagnosis of gastric neoplasm and its staging.
