颅内表皮样囊肿的CT和MRI诊断 被引量:11

摘要 目的:评估颅内表皮样囊肿的CT和MRI表现。材料和方法:47例经手术和病理证实的表皮样囊肿中,10例行CT平扫和增强,其中9例同时行MRI检查;另有37例作常规MRI检查,其中13例还加作FLAIR序列,10例行增强扫描。结果:肿瘤位于桥小脑角25例,鞍区及鞍旁8例,颅中窝底6例,脑内3例,松果作区2例,第四脑室内2例,小脑天幕处1例。47例中不规则形30例,圆形或类圆形17例。CT平扫表现为均匀低密度者6例,混合密度者2例,高密度者2例,3例可见钙化,造影后均未见明显强化。MRI上病灶显示T1WI呈低信号,T_2WI呈高信号者38例, T_1WI及T_2WI呈高低混杂信号者4例, T_1WI呈高信号, T_2WI呈高低混杂信号者4例。9例作造影检查无甚强化表现,仅1例呈轻度环状强化。未见脑水肿表现,占位效应甚轻。FLAIR序列病灶信号均有升高。结论:颅内表皮样囊肿的CT和MRI表现较为特征,尤其是MRI能作出术前准确诊断;FLAIR序列有助于鉴别表皮样囊肿和蛛网膜囊肿。 Purpose: To estimate the diagnostic value of CT and MRI of intracranial epidermoid cyst. Materials and methods: 47 patients with epidermoid cyst proven by surgery and pathology had undergone CT and MRI examination. 10 patients were taken pre - and post - enhancement CT scan. 9 of 10 patients were taken MRI examination.Another 37 patients were proceeded with MRI, 13 of 37 patients were examined with FLAIR sequence additionally and 10 patients with Gd - DTPA. Results: 25 tumors located in CPA, 8 in sellar or parasellar region, 6 in the middle cranial bottom, 2 in the fourth ventricle, 2 in pineal region, 3 in parachyma and 1 nearby tentorial; 30 tumors with irregular shape, 17 with oval shape; 38 patients with hypointense signal on T1WI while hyperintense signal on T2WI , 4 patients display inhomogenenous signal on T1 WI and T2WI, and 4 patients were hyperintense on T1WI while inhomogenenous signal on T2WI; no obvious enhencement but one with slight ring enhancement can be seen; there was no striking brain ede- ma or mass effect; signal intensity of tumor elevated on FLAIR sequence. Conclusion: The manifestation of CT and MRI of epidermoid cyst is characteristic, especially MRI can detect it precisely; FLAIR se- quence is helpful to differentiate epidermoid cyst from arachnoid cyst.
