摘要
目的：探讨磁共振胰胆管造影（ＭＲＣＰ）对恶性胆管梗阻病变的诊断价值与限度。材料与方法：分析３４例恶性胆管梗阻病变的ＭＲＣＰ与ＭＲＩ表现，与手术病理和临床诊断对照。采用重Ｔ_２ＷＩ快速自旋回波（ＦＳＥ）序列，非屏气扫描术，将原始图像作最大信号强度投影（ＭＩＰ）三堆（３Ｄ）重建。结果：３４例一次检查成功。３３例图像显示良好。ＭＲＣＰ 对恶性胆管梗阻病变的定位诊断达１００％，与ＭＲＩ结合定性诊断达９４．１２％（３２／３４）。结论：ＭＲＣＰ检查成功率高，诊断价值大，对恶性胆管梗阻定位准确，优于ＣＴ与ＵＳ，与ＭＲＩ结合，对其定性诊断具有较高的准确性。
Purpose: To explore the diagnosis value and limitation of MR cholangiopancreatography (MRCP ) for malignant bile duct of MRCP and conventional MRI were analyzed in 34 patients of malignant bile duct obstruction. The results of MRCP were compared with clinical diagnosis or surgical findings and pathology. Non - breath - hold heavy T_2WI of fast SE sequence was used. The source images were reconstructed by using three- dimensional (3D) maximum intensity- projection (MIP) algorithm. Results: MRCP were succeeded in all 34 petients and pancreaticobiliary ducts were shown satisfaction in 33 cases. The accuracy of MRCP in the detection of the degree and level of malignant bile duct obstruction were 100% and combing with the routing MRI scaning the accuracy for evaluateing the cases of obstruction was 94. 12 % (32/34). Conclusions: MRCP examination had a high successful rate and accurately define the level of malignant bile duct obstruction. MRCP, combining with conventional MRI scans has a higher accuracy in diagnosis of malignant bile duct obstruction.
