摘要
目的探讨顺铂(DDP)、5-氟脲嘧啶(5-FU)、平阳霉素(PYM)联合四氢叶酸(CF),同期放疗治疗中、晚期口腔鳞癌临床价值。方法 90例中、晚期口腔鳞癌患者随机分为A、B两组各50例,A组为DFP方案(DDP+5-FU+PYM)化疗+同期放疗,B组为DFP方案联合CF化疗+同期放疗。结果 A、B两组有效率(RR)分别为80%、93.3%,差异有统计学意义(p<0.05)。主要不良反应为局部色素沉着、白细胞下降及口腔粘膜炎等,毒性反应B组多于A组。结论 DFP方案联合CF化疗同期放疗治疗中、晚期口腔鳞癌,其疗效优于DFP化疗同期放疗,治疗费用低,可作为口腔癌的一线治疗方案推广。
Objective To explore the clinical value of cisplatin（DDP）,5-Fluorouracil（5-FU）,pingyangmycin（PYM） combined with tetrahydrofolate（CF） and concurrent radiochemotherapy in treating mid-term and terminal oral squamous cell carcinoma.Method 90 mid-term and terminal oral squamous patients were divided into group A and group B randomly,with 50 patients in each group,among which group A adopted the therapeutic plan of DFP（DDP＋5-FU＋PYM）chemotherapy＋ concurrent radiochemotherapy,group B adopted the plan of DFP combined with CF chemotherapy＋ concurrent radiochemotherapy.Result The response rate（RR） of group A and B were 80%、93.3% respectively,and the differences were significant（p0.05）.Major adverse reactions were local chromatosis,white blood cell decrease and oral mucositis etc.There were more toxic reactions in group B than in group A.Conclusion Therapeutic plan of DFP combined with CF chemotherapy＋ concurrent radiochemotherapy to treat mid-term and terminal oral squamous cell carcinoma can effectively control local tumors,have satisfactory short-term curative effect,few adverse reactions,less treatment cost,and can be spread as the first choice of treating oral cancer.
