摘要
目的近年来,腹腔镜结直肠手术已广泛开展,文中探讨腹腔镜结直肠癌根治术的入路选择、镜下解剖及操作要点。方法回顾性分析68例腹腔镜直肠癌根治患者的临床资料及手术录像。结果 64例手术成功,4例中转开腹,64例平均手术时间165min,平均出血量120ml,平均住院11d。术后大出血1例,不全性肠梗阻1例,肠管损伤1例,无吻合口漏及切口感染,无围手术期死亡病例。结论腹腔镜直肠癌根治术效果较好;良好的暴露、恰当的入路、正确的层次和熟练的镜下操作是手术成功的关键。
Objective The purpose of this study was to investigate the surgical approach, anatomical characteristics and op- erating essentials of laparoscopic resection for colorectal carcinoma. Methods We retrospectively analyzed the clinical data and op- eration video of 68 cases of rectal carcinoma treated by laparoscopic resection. Results Operations were successful in all the cases, of which 4 were converted to open surgery. The operation time, intraoperative blood loss and hospital stay averaged 165 min, 120 ml and 11 d, respectively. Operative complications included 1 case of hemorrhage, 3 cases of incomplete intestinal obstruction, and 1 ca- ses of intestinal damage. No death, anastomotic leakage and wound infection occurred. Conclusion Laparoscopic resection has a desirable effect in the treatment of colorectal carcinoma, and successful surgery depends on good exposure, appropriate approach, cor- rect anatomical cross-section and skillful laparoscopie operating techniques.
