坎地沙坦酯的时间治疗学对原发性高血压患者的疗效分析 被引量:5

摘要 目的观察坎地沙坦酯不同用药时间对原发性高血压患者昼夜节律及动脉弹性的影响。方法入选的101例非杓型轻中度高血压患者随机分为两组,晨服组于早8∶00服用坎地沙坦酯4 mg,晚服组于晚8∶00服用坎地沙坦酯4 mg,用药12周。用药前及用药12周后,分别进行24 h动态血压监测,观察两组患者治疗前后血压昼夜节律及踝臂指数的变化。结果两组患者治疗后夜间血压平均值比较差异有统计学意义(P<0.05),晚服组非杓型血压纠正率、晨峰血压降低程度明显高于晨服组(P<0.05),不同时间服药均可提高踝臂指数。结论坎地沙坦酯夜间用药能明显降低高血压患者的晨峰血压,改变非杓型高血压患者的昼夜节律,改善动脉硬化程度。 Objective To evaluate the efficacy of Candesartan Cilexetil chronotherapy on circadian blood pressure profile and arterial elasticity in patients with essential hypertension.Methods 101 patients with non-dipping mild to moderate essential hypertension were randomly assigned to receive Candesartan Cilexetil(4 mg,8 am,morning group or 4 mg,8 pm,evening group) for 12 weeks.24-hour ambulatory blood pressure was performed before drug treatment and at the end of 12 weeks.Circadian blood pressure profile and ankle brachial index were observed.Results There was a significant difference of the average night blood pressure of the two groups after treatment(P 0.05).Non-dipping blood pressure rectification rate and lowering range of peak blood pressure during the morning period in the evening group were remarkably higher than those in the morning group(P 0.05).Ankle brachial index in two groups was improved.Conclusion Candesartan Cilexetil administration in the evening can lower significantly peak blood pressure during the morning period and improve circadian blood pressure profile of non-dipping hypertensive and extent of arterial sclerosis.
