摘要
目的评价碎裂QRS波(fragmented QRS complexes,fQRS)对左心室收缩功能障碍患者预后的预测价值。方法选择2009年7月～2010年12月我院收治的左心室收缩功能障碍患者267例,通过常规心电图检测,将以上患者分为宽QRS组(121例)、fQRS组(59例)、非fQRS组(87例),随访比较三组患者全因死亡率、因心脏疾病再住院次数及室性心动过速的发生情况。结果随访12～30个月,宽QRS组、fQRS组、非fQRS组全因死亡率分别为8.3%(10例)、10.2%(6例)、2.3%(2例),fQRS组死亡率明显高于非fQRS组,差异有统计学意义(P<0.05)。宽QRS组、fQRS组、非fQRS组再次因心脏疾病住院次数分别为192次、99次、113次,宽QRS组、fQRS组再住院次数高于非fQRS组,但差异无统计学意义(P>0.05)。宽QRS组及fQRS组室性心动过速发生率明显高于非fQRS组,差异有统计学意义(P<0.05)。结论 fQRS对左心室收缩功能障碍患者的死亡率及室性心动过速的发生有预测价值。
Objective To evaluate the prediction of fragmented QRS complexes （fQRS） in patients with left ventricular dysfunction. Methods 267 patients in our hospital from July 2009 to December 2010 with left ventricular dysfunction were analyzed. All the patients were divided into three groups, the wide QRS （wQRS） group （n = 121）, the fQRS group （n = 59） and the non-fragmented QRS （non-fQRS） group （n = 87）.The rate of all-cause mortality, the time of re-hospitalization and the occurrence of ventricular tachyarrhythmia were followed-up and compared. Results During a follow-up of 12-30 months, the rates of all-cause mortality in wQRS group, fQRS group and non-fQRS group were 8.3% （10 cases）, 10.2% （6 cases） and 2.3% （2 cases）, the rate of all-cause mortality in fQRS group were found higher than that in non-fQRS group, the difference was statistically significant （P 〈 0.05）. The times of re-hospitalization in wQRS group, fQRS group and non- fQRS group were 192, 99 and 113, the times of re-hospitalization in wQRS group, fQRS group were found higher than those in non-fQRS group, but the differences were not statistically significant （P 〉 0.05）. The occurrence rates of ventricu- lar tachyarrhythmia in wQRS group and fQRS group were found higher than those in non-fQRS group, the differences were statistically significant （P 〈 0.05）. Conclusion The fQRS has a predictive value for the mortality and ventricular tachycar- dia in patients with left ventricular dysfunction.
