摘要
目的探讨直肠全系膜切除术在中低位直肠癌手术治疗中的临床效果,为更好地应用直肠全系膜切除术提供依据。方法将我院肿瘤科2002年3月～2009年3月收治的250例中低位直肠癌患者的临床资料作为研究对象,其中130例行直肠全系膜切除术的患者为观察组,120例未行直肠全系膜切除术的患者为对照组,比较分析两组患者的术中出血量、术后复发率、术后生存率、吻合口漏以及并发症的情况。结果观察组在术中出血量(>100 mL)、1年内复发率、3年内生存率、性功能障碍及排尿功能障碍方面明显优于对照组,两组比较差异均有统计学意义(P<0.01),而两组在吻合口漏、排便功能障碍方面比较差异无统计学意义(P>0.05)。结论直肠全系膜切除术能有效减少中低位直肠癌患者的术中出血量、术后复发率、性功能障碍及排尿功能障碍,有效提高患者的生存率,是临床手术治疗中低位直肠癌的首选方法。
Objective To investigate the rectum total mesorectal resection middle and low rectal cancer surgery in clinical effect, provide a basis for better application of total mesorectal excision. Methods Clinical data of 250 cases of low rectal cancer patients admitted to hospital oncology from March 2002 to March 2009 for the study, 130 cases of the routine rec- tum total mesorectal resection in the observation group, 120 patients did not total mesorectal excision of rectal surgery in the control group, comparative analysis of two groups with intraoperative blood loss, the recurrence rate, survival rate, con- sistent with a mouth leak and complications. Results The observation group in intraoperative blood loss （〉 100 mL）, the re- currence rate within 1 year, 3 years survival rate of sexual dysfunction and voiding dysfunction was significantly better than the control group, the difference between two groups were statistically significant（P 〈 0.01 ）, the two groups in anastomotic leakage, bowel dysfunction difference was not statistically significant （P 〉 0.05）. Conclusion Total mesorectal resection conclusion rectum can effectively reduce the amount of hemorrhage in patients with low rectal cancer, the recurrence rate, sexual dysfunction and voiding dysfunction, and effectively improve the survival rate is low rectal cancer in clinical surgery the preferred method.
