摘要
目的探讨选择性剖宫产和阴道分娩两种不同分娩方式对产后盆底组织功能的影响。方法选择2009年5月～2010年5月在我院分娩的产妇62例,按随机分组法分为阴道分娩组(n=32例)和选择性剖宫产组(n=30例)。分别分娩后3个月静息状态下检测产妇生殖裂孔的LHD、LHAP、LHLR、LUG的变化,并对Oxford骨盆底肌力评分,对两组产妇盆底肌力情况进行比较。结果选择性剖宫产组分娩后3个月静息状态下生殖裂孔的LHD、LHAP、LHLR、LUG分别为(5.01±0.61)cm、(5.11±0.77)cm、(3.81±0.22)cm、(1.92±0.26)cm,与阴道分娩组相比较,差异有统计学意义(P<0.05)。选择性剖宫产组盆底肌力评分≥4分者占83.33%,阴道自然分娩组占53.12%,两组比较差异有统计学意义(P<0.05)。结论妊娠生理变化可以引起盆底组织的损伤,选择性剖宫产对盆底损伤小,较阴道分娩对盆底具有一定的保护作用。
Objective To explore selective cesarean section and vaginal delivery of two different ways childbirth with post- partum pelvic floor the effect of organizational function. Methods Selecteel in May 2009 to May 2010 in our hospital de- livery of 62 cases of women, according to the randomized method into a vaginal delivery group （n -- 32 cases） and selec- tive cesarean sections （n = 30 cases）. After the birth of 3 months respectively the resting state detection puerpera repro- ductive stage, LHD LHAP, LHLR, LUG changes,and Oxford pelvic bottom muscle strength rating, in two groups at the situation of women muscle strength comparison. Results Three months resting reproductive hole LHD LHAP, LHLR, LUG after delivery of selective cesarean section group were （5.01_＋0.61） cm, （5.11_＋0.77） cm, （3.81-＋0.22） cm, （1.92＋0.26） cm, and the vaginaldelivery group,the difference was statistically significant （P 〈 0.05）. Selective cesarean section group pelvic floor muscle strength score of 3 accounted for 83.33% of spontaneous vaginal delivery group accounted for 53.12%, the difference of the two groups was statistically significant （P 〈 0.05）. Conclusion Physiological changes of pregnancy can cause pelvic floor damage, selective cesarean section for pelvic floor injury is small, compared with vaginal delivery on the pelvic floor has a protective effect.
