老年男性2型糖尿病患者甲状腺功能及甲状腺免疫功能研究 被引量:6

摘要 目的探讨老年男性2型糖尿病(T2DM)患者甲状腺功能及甲状腺免疫功能变化及临床意义。方法对比观察组(n=100)和对照组(n=100)甲状腺功能指标、甲状腺免疫功能指标、糖代谢指标和脂代谢指标并进行分析。结果观察组T4水平较对照组显著降低,TSH水平显著升高(P<0.05);观察组aTG、aTPO水平均显著高于对照组(P<0.01)。T2DM患者TSH水平与BMI、病程、aTG呈正相关(P<0.05),与T3、FT3、T4、FT4呈负相关(P<0.05)。aTG、aTPO水平呈正相关(P<0.05)。aTG阳性和阴性组在病程、BMI、TG、TC、HDL-C、LDL-C、T3、FT3、T4、FT4、TSH差异有统计学意义(P<0.05),aTG水平与病程、BMI、TSH呈正相关,与T3、FT3、T4、FT4呈负相关(P<0.05)。结论老年男性T2DM患者存在甲状腺功能和甲状腺免疫功能异常,以T3、T4降低、TSH升高为主要表现,甲状腺自身抗体升高,病程和BMI与之关系密切。 Objective To investigate the thyroid function and thyroid immune function and clinical significance of alder men with type 2 diabetes(T2DM). Methods Compared the patient group(n = 100) and the control group(n = 100) index of thyroid function, thyroid immune function indicators, analyzed indicators of glucose metabolism and lipid metabolism in- dicators. Results The patient group T4 level was significantly reduced compare with the control group, and TSH level was significantly increased(P 〈 0.05 ); aTG and aTPO of patient group were significantly higher than that of control group(P 〈 0.01 ). TSH levels were positively correlated with BMI, disease duration and aTG(P 〈 0.05), and negatively correlated with T3, FT3 ,T4and FT4. aTG and aTPO levels were positively correlated (P 〈 0.05). The difference of course of the disease, BMI, TG ,TC, HDL-C, LDL-C, T3, FT3, T4, FT4, TSH and aTG between positive and negative groups were statistically significant (P 〈 0.05), aTG level was positively correlated with the course of disease, BMI and TSH, and was negatively correlated with T3 and FF3, T4, and FT4(P 〈 0.05). Conclusion The abnormal thyroid function and thyroid immune func- tion in elderly male patients with T2DM, T3 and T4 decreased and TSH increased as the main performance, elevated thy- roid autoantibodies, with closely related to disease duration and BMI.
