摘要
目的观察瑞舒伐他汀对老年高血压患者主动直立试验(AOT)时脉压变化(△PP)的影响,探讨降低脉压变化对老年高血压患者心血管病危险度和预后的临床价值。方法选择100例AOT时脉压变化的老年高血压患者,随机分为瑞舒伐他汀治疗组(n=50例)和常规对照组(n=50例),治疗组在对照组的基础上每晚口服瑞舒伐他汀10 mg,治疗6个月,观察瑞舒伐他汀对△PP、hs-CRP、IMT、PS的影响,并进行相关性分析。结果与对照组比较,治疗后治疗组△PP降低26.85%(P<0.01);hs-CRP降低34.35%(P<0.01);IMT降低15.05%(P<0.01),PS降低46.39%(P<0.01)。多元逐步回归分析,AOT时脉压变化和hs-CRP、IMT、PS呈显著正相关(F值分别=7.24、6.48、5.29,P均<0.01)。结论瑞舒伐他汀能显著降低AOT时的脉压变化,降低老年高血压患者心血管危险性,改善预后;脉压变化与hs-CRP、IMT、PS的变化密切相关。
Objective To observe the effect of rosuvastatin on changes of pulse pressure in elderly patients with hyperten- sion and atherosclerosis risk factors, explore the clinical value of rosuvastatin on the risk of cardiovascular disease and e- valuation of prognosis during active orthostatic test （AOT）. Methods A total of 100 elderly patients with hypertension were selected by changes of pulse pressure （increase or reduce） during active orthostatic test. All patients were randomly divided into two groups （50 cases of commom treatment plus rosuvastatin treatment group, 50 cases of common treatment group）. Patients in treatment group took rosuvastatin based on the conventional treatment every night for six months. Then the pa- rameters of rosuvastatin on changes of pulse pressure, hs-CRP, IMT, PS were observed and the correlation between changes of pulse pressure and clinical parameters was analyzed. Results Compared with patients in control group, after rosu- vastatin treatment, APP reduce by 26.85% （P 〈 0.01）, hs-CRP reduce by 34.35% （P 〈 0.01）, IMT reduce by 15.05% （P 〈 0.01）, PS reduce by 46.39% （P 〈 0.01）. Multiple regression analysis showed that there was significant positive correlation hetween the blood pressure and hs-CRP, 1M^E PS respectively during AOT （F= 7.24, 6.48, 5.29, P 〈 0.01）. Conclusion Ro- suvas/atin can reduce change of pulse pressure in elderly patients with hypertension during AOT, decrease hs-CRP level aml IMT, so can reduce cardiovascular disease risk of ehlerly patients with high hlood pressure and improve the prognosis.
