早期肠内营养对机械通气老年患者的临床作用 被引量:5

摘要 目的早期肠内营养对机械通气老年患者的临床作用。方法按是否在机械通气48h内开始肠内营养支持治疗,将56例老年机械通气患者分为早期肠内营养组38例和非早期肠内营养组28例,在摄入相当热量和氮量的条件下观察两组患者机械通气时间、平均住院时间、发生呼吸机相关性肺炎(VAP)的病例数和死亡病例数、出院时/死亡前APACHEⅡ评分。结果早期肠内营养组和非早期肠内营养组机械通气时间、平均住院时间、VAP病例数、死亡病例数分别为(20.5±8.6)d/(32.4±11.0)d、(37.4±16.5)d/(66.7±26.2)d、25例/21例、11例/12例,经统计学处理差异有显著性(P<0.05);而APACHEⅡ评分为(22.7±12.9)分/(24.5±15.3)分,经统计学处理差异无显著性(P>0.05)。结论早期肠内营养能明显缩短老年机械通气患者机械通气时间和住院时间,降低呼吸机相关性肺炎的发生率和病死率。 Objective To study the effect of the early enteral nutritionon the elderly patients with mechanicalventilation. Methods 56 elderly patients with mechanical ventilation were classified into two groups., group EEN included 38 patients receiving enteral nutrition within 48 hours of the initiation of mechanical ventilation and group non--EEN included 28 patients which did not receive the early enteral nutrition in 48 hours. Day s of mechanical ventilation and inhospital, patients with VAP, mortality and acute physioloy and chronic health evaluation Ⅱ ( APACHE Ⅱ ) were compared. Results Days of me- chanical ventilation and in hospital, patients with VAP, mortilaty of group EEN and non--EEN were (20.5±8.6) d/ (32.4±11.0) d, (37.4±16.5) d/ (66.7±26.2) d, 25 ca ses/ 21 cases, 11 cases/ 12 cases respectively (P 〈 0.05) ; but APACHE Ⅱ was (22.7± 12.9) / (24.5±15.3) (P 〉 0.05). Con- clusions The early enteral nutrition has obviously shortened the days of mechanical ventilation and in- hospital, reduced the incidence of ventilator--associated pneumonia and mortality of the elderly with me- chanical ventilation.
