同轴微切口超声乳化治疗老年性白内障临床分析 被引量:7

摘要 目的探讨同轴微切口及小切口超声乳化白内障吸除联合人工晶体植入术治疗老年白内障的临床效果。方法对我院自2006年4月—2011年12月采用超声乳化白内障吸除联合人工晶体植入手术治疗老年白内障192例(192眼)的临床资料进行回顾性分析,其中微切口组87例(87眼),小切口组105例(105眼)。对两组患者术后的矫正视力及手术源性散光进行比较。结果两组患者手术中所用平均超声能量(AVE)和有效超声乳化时间(EPT)及术中前房稳定性比较无统计学差异(P>0.05);微切口组术后第1、3天角膜水肿比小切口组轻,差异有统计学意义(P<0.05),术后第7天更轻微,差异有显著统计学意义(P<0.01);微切口组术后第1、7、28天视力恢复情况均好于小切口组、术后手术源性散光度(SIA)均小于小切口组,差异有统计学意义(P<0.05)。结论同轴微切口超声乳化白内障吸除联合人工晶体植入术后角膜水肿发生率低、视力恢复的好并能够有效地减少SIA的发生。 Objective To analyze the clinical effect of coaxial micro-phacoemulsification or coaxial small incision phacoemulsification combined with intraocular lens implantation on senile cataract.Methods Form April 2006 to December 2011,the clinical data of 192 cases of phacoemulsification combined with intraocular lens implantation on senile cataract were retrospective analyzed.There are 87 cases of micro incision and 105 cases of small incision.Comparing the eyesight correction and surgically induced astigmatism between patients of two groups.Results The using of AVE and EPT and the stability of anterior chamber in operation between 2 groups has no statistic difference(P0.05).The postoperative corneal edema of the micro incision group in day 1 and 3 after operation was milder than the small incision group and the difference has statistical significance(P0.05).It was much milder in 7 days after operation and the difference has remarkable statistical significance(P0.01).The visual acuity rehabilitation and the SIA after operation of the micro incision group in day 1,3 and 7 after operation were better than the small incision group and the difference has statistical significance(P0.05).Conclusion The coaxial micro-phacoemulsification combined with intraocular lens implantation can reduce the rate of postoperative corneal edema and SIA also well in visual acuity rehabilitation.
