儿童病毒性脑炎的氢质子磁共振谱及其对预后评估的价值 被引量:1

摘要 目的探讨儿童病毒性脑炎时氢质子磁共振波谱(1H MRS)的表现及其对病情预后的评估价值。方法回顾分析经我院儿科确诊的20例儿童病毒性脑炎的临床资料,按格拉斯哥昏迷评分(GCS)分为两组:重型(GCS 3~8分)8例、轻型(GCS 9~15分)12例,所有患儿和对照组10例均行核磁共振(MRI)扫描,并以病灶区(MRI检出信号异常处)及相对应的正常区域为感兴趣区(ROI)进行1H MRS分析,且随访1年后行智力测验。结果病灶区1H MRS结果均有异常,表现为乙酰天门冬氨酸盐(NAA)下降、胆碱(Cho)升高,NAA/Cho降低,重型组该比值降低更明显,且与智商降低呈正相关。结论儿童病毒性脑炎时1H MRS的表现可作为判断脑损伤的标准,对病情的预后评估具有重要价值。 Objective To explore the manifestation of proton magnetic resonance spectroscopy(1H MRS) in children with viral encephalitis and evaluate its value to the prognosis.Methods Twenty children with viral encephalitis confirmed clinically were retrospectively analyzed.They were divided into two groups according to the Glasgow Coma Scale(GCS): 8 severe cases in 3-8 scores,12 mild cases in 9-15 scores.10 cases were as controls without nervous system diseases.All the children were experienced with 1H MRS focus on the region of interest(ROI) which involved in abnormal areas detected by magnetic resonance imaging(MRI) and those corresponding normal areas.After 1 year of follow-up,intelligence tests were dong to them.Results In all the abnormal areas,1H MRS showed that N-acetylaspartate(NAA) decreased and Choline(Cho) increased which led to the drop of NAA/Cho.The more severe of the disease,the lower the ratio would be.Moreover,there was a positive relationship between this ratio and the intelligence quotient.Conclusion The manifestation of 1H MRS in children with viral encephalitis may be a criterion of neuronal damage and have high value in the evaluation of the prognosis.
