摘要
目的探讨螺旋CT三维重建技术对颌面部骨折诊断的应用价值。方法回顾性分析22例经手术治疗的颌面部骨折病人的X线平片、二维CT片及螺旋CT三维重建图像:表面阴影显示法(SSD)、容积再现(VR)、最大密度投影重建(MIP)等显示方法,对三种检查方法进行比较。结果 22例颌面部骨折病人经临床手术证实的42处骨折,螺旋CT三维重建图像可显示41处,但有1处骨折显示不清晰,准确率达到95.24%,显著高于二维CT的76.19%(P<0.05)和X线平片的59.52%(P<0.01)。结论相对于X线平片和二维CT,螺旋CT三维重建技术能够多方位、立体、全面、更加清晰的显示患者颌面部骨折部位及走势,对颌面部骨折的准确诊断以及相应的治疗方案确认具有较高的临床指导价值。
Objective To investigate the application value of spiral CT three-dimentional reconstrucion technology in the diagnosis of maxillofacial fractures.Methods The X-ray,two-dimensional CT and spiral CT three-dimensionally reconstructed images of 22cases who had undergone surgery for maxillofacial fractures were retrospectively analyzed.Three examination methods-shaded surface display(SSD),volume rendering(VR),maximum intensity projection(MIP)were used to compare these images.Results Clinical operation confirmed that there were 42fractures among the 21cases and the CT three-dimensionally reconstructed images could display 41,with only one fracture not clear.So the accuracy rate reached to 95.24%,which was significantly higher than those of two-dimensional CT(76.19%,P 0.05)and X-ray films(59.52%,P 0.01).Conclusion Compared with the X-ray and two-dimensional CT,spiral CT three-dimensional reconstruction technology can display the maxillofacial fracture sites and shapes in a multi-dimentional,allround and clearer way.It has significant clinical value for guidance to accurate diagnosis and appropriate treatment of maxillofacial fracture.
