摘要
目的:通过经兔耳静脉持续输入静脉营养液(INA),观察不同渗透浓度的INA对静脉炎发生的影响。方法:将24只实验兔随机分成三组,即低渗透浓度组(300~590 mmol/L)、中渗透浓度组(600~890 mmol/L)和高渗透浓度组(1 000~1 350 mmol/L),每组8只。经兔耳静脉以10 ml/(kg.h)的速度持续输入INA 12 h,输液结束24 h后,获取兔耳静脉标本,并行组织病理学检查。结果:低渗透浓度组兔病理学检查出现静脉炎样改变较少;中渗透浓度组出现轻度血管内皮细胞脱落、炎性细胞浸润、血管周围组织水肿伴少量血栓形成;高渗透浓度组出现严重血管内皮细胞脱落、大量炎性细胞浸润、血管周围组织严重水肿伴血栓形成等血管损伤的表现。结论:高渗透浓度INA会导致血管损伤,INA渗透浓度与静脉炎的发生有着密切的关联。
Objective: The venous phlebitis is one of the most common complication of peripheral parenteral nutrition(PPN).This study aimed to investigate the effects of different osmotic pressure in the development of phlebitis on rabbit ear vein through continuous infusion of intravenous nutrition admixture(INA).Methods: 24 New Zealand white rabbits were divided into three groups:hypotonic INA(300~590 mmol/L),mid-osmotic INA(600~890 mmol/L),and hypertonic INA(1 000~1 350 mmol/L).INA solution was infused into rabbit ear veins at 10 ml/(kg·h)for 12 hours.After 24 hours from the end of the infusion,the infused veins were sampled and examined histopathologically.Results: Fewer phlebitic changes were presented in the veins in hypotonic group.In mid-osmotic group,there were mild endothelial cells shedding,inflammatory cells infiltration,and obvious perivascular swelling with fewer thrombus.The venous lesion,such as severe endothelial cells shedding,increased inflammatory cells infiltration,significant perivascular edema and thrombosis,were observed in hypertonic group.Conclusion: Osmotic pressure of INA solution plays a critical role in the development of phlebitis and hypertonic solution results in venous lesion.
