高压氧结合神经节苷酯治疗新生儿缺血缺氧性脑病效果分析 被引量:11

摘要 目的:研究高压氧结合神经节苷脂治疗新生儿缺血缺氧性脑病(Hypoxic-ischemic encephalopa-thy,HIE)的疗效和机制。方法:96例HIE新生儿随机分为实验组和对照组,对照组采用常规治疗,实验组在对照组的基础上采用高压氧结合神经节苷脂治疗。观察治效、疗后症状体征改善程度、新生儿行为神经评分(Neonatal behavioral neurological assessment,NBNA)和血清神经元特异性烯醇酶(Neu-ron specific enolase)浓度。结果:实验组的治疗有效率为95.83%,而对照组的治疗有效率为72.91%,实验组和对照组治疗有效率之间的差异具有统计学意义(χ2=9.56,P<0.05)。实验组的意识障碍恢复时间、惊厥停止时间、原始反射正常时间和肌张力正常时间均短于对照组,并且均具有统计学意义(P<0.05)。实验组与对照组在治疗后的NBNA评分均较治疗前提高,并且均具有统计学意义(P<0.05);治疗后实验组NBNA评分高于对照组,并且具有统计学意义(P<0.05)。实验组和对照组治疗后的血清NSE浓度均较治疗前减少,差异具有统计学意义(P<0.05)。治疗后实验组治疗后血清NSE浓度较对照组减少,差异具有统计学意义(P<0.05)。结论:使用高压氧和神经节苷酯治疗新生儿缺血缺氧性脑病能够更好修复脑细胞损伤来提高治疗的有效率。 Objective: To investigate the clinical efficacy and mechanism of hyperbaric oxygen com- bined with ganglioside in treatment of neonatal hypoxic ischemic encephalopathy (HIE). Methods: A total of 96 HIE neonatal were randomly divided into the experimental group and control group. The control group was treated with conventional treatment, while the experimental group was treated with hyperbaric oxygen combined with ganglioside. The recovery time of conscious disturbance, the stop time of convul- sions, recovery time of primitive reflexes and muscle tension of experimental group were significantly shorter than that of the control group (P〈0.05). Results: The clinical efficiency of experimental group (95.83%) was significantly higher than that of the control group (72.91%) (X2= 9.56, P〈0.05). NBNA score was significantly increased in both two groups after treatment (P〈0. 05), and the score was significantly higher in experimental group(P〈0.05). NSE concentration was significantly reduced in both groups(P〈0.05), and it was significantly lower in experimental group(P〈0.05). Conclusions. The use of hyperbaric oxygen and ganglioside is effective in treatment of neonatal hypoxic ischemic encephalopathy. It can repair neuron cells injury to improve treatment efficiency.
